Objective1.To explore the cognition of critical care specialist nurses on job crafting to improve work engagement;2.To construct an intervention program to improve the work engagement of critical care specialist nurses based on the job crafting theory.Methods1.Qualitative interview:Using the purposive sampling method,a total of 12 critical care specialist nurses from Class Ⅲ Grade A general hospitals in Jiangsu Province were selected as the research objects from April to July 2022.Semi-structured interviews were used to investigate and understand the cognition of critical care specialist nurses on job crafting so as to provide a basis for the construction of later intervention plans.2.Literature analysis:According to literature analysis,to understand the research status of nurses’ work engagement intervention based on job crafting,and to construct the basic framework of intervention programs.3.Delphi expert consultation:based on the interview results,the intervention program was adjusted and improved.From September to December 2022,27 experts in the field of nursing psychology,nursing management and critical care were selected as the consultation objects by the combination of email consultation and wechat consultation.The expert consultation form was used to conduct two rounds of consultation.The expert opinions were sorted out,analyzed,fed back and revised,and the intervention plan was finally determined.Results1.The results of qualitative interviews showed 4 themes and 13 sub-themes of job crafting experience of critical care specialist nurses.There were four themes of task crafting,cognitive crafting,relationship crafting and influencing factors of job crafting.Among them,task crafting included three sub-themes of work process crafting,work method crafting and scientific research innovation,and cognitive crafting included two sub-themes of role cognition crafting and professional values crafting of specialist nurses.Relationship crafting included three sub-themes:doctor-nurse relationship crafting,nurse-patient(family)relationship crafting and nurse-nurse relationship crafting.The influencing factors included five sub-themes:physical environment,personal factors,department atmosphere,leadership style and communication platform.2.The average age of the experts was 47~57(52.48±5.49)years old.The average working years were 26~39(33.30±6.67)years.Most of them(77.80%)had bachelor’s degree,and 74.10%had senior titles and 25.90%had associate senior titles.The authority coefficient of expert consultation was 0.87.The Kendall coordination coefficients of the two rounds were 0.09 and 0.133,respectively,and the differences were statistically significant(P<0.001).After two rounds of expert consultation,the coefficient of variation of the importance of each index ranged from 0.04 to 0.16,all of which were less than 0.25.Combined with the results of qualitative interviews and literature analysis,the work engagement intervention program for critical care specialist nurses was finally established,including four parts:intervention objectives,intervention contents,intervention forms and intervention effect evaluation,4 first-level indicators,18 second-level indicators and 60 third-level indicators.ConclusionThe work engagement of critical care specialist nurses is at a medium level,and certain intervention measures can be taken to improve their work engagement.Literature analysis showed that job remodeling intervention is one of the possible effective methods.Therefore,based on the interview results ofj ob remodeling of specialist nurses in critical care and based on job remodeling theory,this study used Delphi method to construct an intervention program to improve the work engagement of specialist nurses in critical care,in order to provide guidance and theoretical basis for the management of clinical critical care specialist nurses.
