Objective:1.To understand experts’ suggestions on the role characteristics,knowledge and skills of long-term care workers at different levels,and to provide reference for determining the grading index of care workers;2.To clarify the role and work content of care workers at different levels in long-term care and the grading requirements and standards and demonstrate the index system by delphi;3.To construct a grading evaluation index system suitable for long-term care workers in China and provides reference for the admission,selection,assessment,training,improvement of the professional ability and quality,team construction and development mode of care workers.Methods:1.Preliminary research on the basis of long-term staff classificationThrough literature review,the current situation of training and grading of long-term care workers at home and abroad was sorted out and summarized,the interview outline was determined,and the qualitative research method was adopted to conduct semistructured interviews with 16 experts in the field of geriatric care.To investigate and analyse the experts’ views on the training status of long-term care personnel and the suggestions on forming the evaluation basis of nursing personnel classification which fit the characteristics of long-term care in our country.2.Construction of grading evaluation index system for care workersBased on the results of literature review and qualitative research,a framework of grading evaluation index system for care workers was constructed,and a draft of grading evaluation index system for care workers was formed to meet long-term care needs and career development by refining the results of the characteristics of each level of the experts in the semi-structured interview as a supplement.The delphi expert consultation method is used to demonstrate and revise the grading evaluation index system of care workers.3.Weighting quantification of indicators and verificating the grading evaluation index system for care workers(1)Building a hierarchical model for grading evaluation indicators of care workers and quantify the weightThe analytic hierarchy process(AHP)was used to establish the hierarchical model of evaluation indicators and verify the internal consistency.The weights of indicators at all levels were set and the qualitative data were converted into quantitative data to enhance the feasibility and operability of the evaluation index system.(2)Preparing self-evaluation questionnaire for care workers for preliminary verific ationBased on the grading evaluation index system of nursing staff,the self-evaluation que stionnaire of care workers was prepared.The objective sampling method was adopted to se lect care workers working in the designated institutions participating in long-term care insu rance in Suzhou for pre-investigation,and the reliability and validity of the questionnaire w ere analyzed statistically,so as to further test and demonstrate the index system.Results:1.The grading evaluation index system of nursing staff consists of three levels:care worker,care worker-in-charge and care worker supervisor,and consists of five first-level indicators,including theoretical knowledge,practical skills,comprehensive ability,professional attitude and personal characteristics,53 second-level indicators and 217 thirdlevel indicators.2.In the first round of expert consultation,22 questionnaires were issued and 22 were recovered.The expert positive coefficient was 100%,the authority coefficient was 0.863,and the coordination coefficient was 0.064.A total of 22 questionnaires were sent out in the second round of expert consultation,and 21 were recovered.The expert positive coefficient was 95.45%,the authority coefficient was 0.862,and the coordination coefficient was 0.171.The coordination coefficient of the two rounds of expert consultation opinions had P<0.05,and the results were statistically significant.3.According to the weight analysis of the importance score of each index by experts,the consistency ratio of the first and second indexes ranges from 0 to 0.0108,all the results less than 0.1,meeting the consistency requirements;According to the weight value,the first level index was ranked as theoretical knowledge(0.2599),practical skill(0.2599),professional attitude(0.2599),comprehensive ability(0.1382)and personal trait(0.0820).The evaluation index system was verified through a small scale survey,and 105 care workers were graded and questionnaires corresponding to different levels were issued for investigation.Seven evaluation items at the supervision level were deleted through the preliminary questionnaire analysis;The Cronbach a coefficient of the evaluation index system was 0.995,0.994 and 0.990,respectively.The Cronbach a coefficient of the primary index was between 0.952 and 0.990,the index of item content(I-CVI)was 0.952 to 1.000,and the S-CVI/Ave was 0.990.The correlation coefficient between the primary index and the overall index ranged from 0.685 to 0.976,with P<0.01.The results were statistically significant,and the evaluation index system had good reliability and validity.Conclusion:Through literature review,semi-structured interview and Delphi method,this study constructed a hierarchical evaluation index system for care workers,including three levels of care worker,care worker-in-charge and care worker supervisor,five first-level indicators including theoretical knowledge,practical skills,comprehensive ability,professional attitude and personal traits,53 second-level indicators and 217 third-level indicators.By using analytic hierarchy process(AHP)and questionnaire survey,the index system was verified to be scientific,rational and practical,and the reference basis was provided for the access,evaluation,assessment,training and skill identification of Chinese long-term care care workers in the future,so as to improve the professionalism and skills of care workers,and promote the sustainable development of care team.
