Research On The Construction Of Diversified Caring Bank Framework System And Management Strategy Based On Long-term Care Insurance System

Objectives:1.To define the elements of a diversified caring bank;2.To construct a scientific and reasonable framework system and contents for the diversified caring bank;3.To explore the management strategy of the diversified caring bank suitable for disabled older adults,and provide a scientific decision basis for establishing a caring system suitable for the rapidly increasing aging population.Content and Methods:This study is divided into three parts:1.Defining the elements of a diversified caring bank based on the long-term care insurance systemDomestic and foreign databases were systematically searched,and relevant literature on the application of time banking in elderly care services was collected.26 literatures were analyzed using the literature analysis method and a structural frame of the elements of a diversified caring bank was initially formed.Additionally,15 nursing administrators of pension institutions and long-term care insurance policy implementation administrators in Suzhou were interviewed using a semi-structured format to obtain the opinions of experts on the elements of a diversified caring bank.The first draft of the elements of a diversified caring bank was formed by combining the results of literature research and the suggestions from the interviews.Using an expert meeting method,12 multidisciplinary experts from universities,pension institutions,and long-term care insurance policy management and implementation departments were invited to hold two expert meetings to consult on the draft of elements,resulting in the improvement and definition of the elements of a diversified caring bank.2.Constructing the framework system and contents of the diversified caring bank based on the long-term care insurance systemOn the basis of the elements of a diversified caring bank,10 experts from universities,pension institutions,and long-term care insurance policy management and implementation departments were organized to carry out two brainstorming meetings to initially construct the framework system and contents of a diversified caring bank through interdisciplinary cooperation.Using the Delphi method,21 experts in the field of elderly care were invited to conduct two Delphi expert consultations to demonstrate,revise,improve,and finally construct a scientific and reasonable framework system and contents for a diversified caring bank.3.Exploring the management strategy of the diversified caring bank based on long-term care insurance systemOn the basis of the elements,framework system,and contents of a diversified caring bank,9 experts from universities,pension institutions,and long-term care insurance policy management and implementation departments were organized to carry out two brainstorming meetings to initially draw up the management strategy of a diversified caring bank from multidisciplinary perspectives.Using the Delphi method,21 experts in the field of elderly care were invited to conduct two expert consultations to qualitatively evaluate the preliminary management strategy and finally formulate a multi-faceted and multi-level management strategy.Finally,the analytic hierarchy method was used to construct the hierarchical structure model of the management strategy and verify its consistency and rationality,and scientifically set the weight of each level of the management strategy.Results:1.Defining the elements of a diversified caring bank based on the long-term care insurance systemBased on the literature analysis of 26 domestic and foreign literatures on the application of time banking in elderly care,interviews with 15 nursing administrators of Suzhou pension institutions,and long-term care insurance policy implementation managers,and expert meetings,specific and clear elements were defined,including personnel elements,training elements,caring elements,storage elements,exchange elements,and management elements.2.Constructing the framework system and contents of the diversified caring bank based on the long-term care insurance systemBy using brainstorming and the Delphi method to qualitatively evaluate each element of the framework system and contents,a scientific and comprehensive framework system and contents for the diversified caring bank were developed.The framework system includes three modules:diversified caring staff(consisting of 6 items),diversified caring contents(comprised of 44 items),and diversified storage and exchange contents(which includes 8 items).3.Formulating the management strategy of the diversified caring bank based on the long-term care insurance systemUsing the elements and framework system,a management strategy for the diversified caring bank based on the long-term care insurance system was developed.Brainstorming and the Delphi method were used to formulate and verify the management strategy.The management strategy includes 6 modules:recruitment management(consisting of 9 items),training management(14 items),caring management(10 items),storage management(6 items),exchange management(12 items),and security management(14 items).The hierarchical structure model of the management strategy was developed using the analytic hierarchy process,and weights were assigned to all levels.The first-level items were assigned the following weights:recruitment management(0.0745),training management(0.0745),caring management(0.1906),storage management(0.2655),exchange management(0.1293),and security management(0.2655).In the second-level items,the top 5 items with the highest weights were:storage information management(0.1310),storage type(0.0825),rights and interests protection(0.0793),risk management protection(0.0793),and exchange content setting(0.0753).The consistency ratios of all items were less than 0.1,indicating that the hierarchical structure model of the management policy was reasonable,reliable,and consistent.Conclusion:1.The elements of the diversified caring bank,which were identified through literature research,qualitative research,and expert meetings,are systematic,scientific,and applicable.These elements provide an important basis for the construction of the framework system and management strategy.2.The framework system and contents of the diversified caring bank,developed through brainstorming and the Delphi method,are scientific and reasonable.This framework lays a theoretical foundation for the management strategy and plays an important role in promoting the implementation of the diversified caring bank.3.The management strategy of the diversified caring bank,developed through brainstorming and the Delphi method,is reliable,scientific,and operational.It provides a scientific basis for decision-making and for establishing a caring system suitable for the rapid increase of the aging population in our country.
