Objective:1.On the basis of the current connotation of nursing interruption events,this study constructed the operation room nursing interruption event questionnaire.2.To observe the occurrence of nursing interruption events in the operating room,analyze their specific contents,explore the factors related to the outcome of the interruption and nurses’ coping style,and analyze the observed nursing hidden dangers,so as to provide support for continuous clinical quality improvement and improvement of nursing safety in the operating room.Methods:1.Literature analysis and Delphi expert consultation were used to determine the nursing interruption incident questionnaire in the operating room.2.From December 2021 to September 2022,data was collected from 5 hospitals in Changzhou using the operating room nursing interruption event questionnaire.Using convenience sampling method to select partial endoscopic surgery and open surgery as research subjects,collect nursing interruption events that occurred during the surgery.Analyze the specific content of interruption events,as well as the related factors of interruption outcome and nurses’ coping style,and analyze and classify the hidden dangers caused by the interruption of nursing work.SPSS25.0 was used for statistics and analysis.t test of two independent samples was used for inter-group comparison of measurement data,Chi-square test was used for inter-group comparison of counting data,and Bonferroni test was used for multiple comparison.Results:1.Form an operation room nursing interruption event questionnaireIn this study,a total of 18 experts completed two rounds of expert consultation,and the questionnaire recovery rates were 90%and 100%,indicating a high degree of experts’activeness.The expert authority coefficient of the two rounds is 0.92,indicating that the expert opinion has a high degree of authority.Kendall’s coordination coefficients were 0.19 and 0.17,respectively,P<0.001,indicating that expert opinions tended to be consistent.2.Current status of operating room nursing interruption eventsDuring the observation period of 640h,2648 nursing interruption events occurred,with an average frequency of(5.53±2.32)times/h.The most frequently interrupted main task was nursing records(n=1028,38.82%),the most frequently interrupted source was the surgeon(n=1133,42.79%),the most frequently interrupted cause was the need for surgery(n=1265,47.77%),and the most commonly used coping mode of nurses was immediate response(n=1804,68.13%).The interruption type was mainly intrusive(n=1502,56.72%),and the interruption outcome was mostly negative(n=2217,83.72%).The interruption duration was mostly<1min(n=1495,56.46%).After interruption,most nurses could return to their primary task(n=2257,85.23%).3.Factors associated with the outcome of care interruption eventsThere were significant differences in the outcome of interruption events among different surgical methods and nurses’ professional titles(χ2=32.440,144.269,P<0.001).There was no significant differences in the outcome of interruption events among different nurses’ age,education and working years(χ2=4.783,2.770,0.225,P>0.05).4.Factors related to nurse coping styles after nursing interruption events.There were significant differences in the coping styles of nurses with different operation methods,nurse age,nurse title,nurse education and nurse working years(χ2=58.908,27.645,40.772,32.505,43.425,all P<0.001).5.The occurrence of nursing safety hazards after the interruption of nursing work.A total of 47 nursing safety hazards occurred,the incidence of hidden dangers is 17.74 per thousand.The most frequently interrupted main task is item counting(n=12,25.53%),followed by tripartite verification(n=9,19.15%);The most common source of interruption was surgical doctors(n=18,38.30%),followed by fellow nurses(n=14,29.79%);The most common reason for interruption was surgical need(n=20,42.55%),followed by help from other nurses(n=7,14.89%);The most commonly used coping methods of nurses were immediate coping(n=29,61.70%);The interrupt type was mainly intrusive(n=25,53.19%).Most nurses can return to the primary task after interruption(n=26,55.32%).Conclusion:1.The questionnaire of nursing interruption in the operating room was formed,including 3 first-level indicators,7 second-level indicators and 28 third-level indicators.The content covers the basic situation in the operating room,the basic content of nursing interruption event and the outcome of nursing interruption event.The index setting is scientific and comprehensive,which can be used for clinical reference.2.Nursing interruption events in the operating room have a high frequency,and the outcome is mostly negative,which should be paid attention to.3.Nursing interruption event outcome is different among different surgical methods and nurse titles.Different surgical methods,nurses’ age,educational background,professional title and working years have different coping styles.4.The hidden danger rate of nursing interruption in operating room is relatively high,mainly intrusion type.At the same time,in the occurrence of hidden dangers,nurses mainly take immediate response,less take delay or refusal to deal with the way.Therefore,nursing managers in the operating room should attach importance to training nurses in coping with nursing interruption in the operating room,improve nurses’ risk awareness,and construct coping strategies for nursing interruption events in the operating room to help nurses correctly identify the interruption and adopt appropriate coping methods,so as to improve patients’ surgical nursing safety.
