| Background: Intrauterine lesions are a general term for a class of diseases,common types include: endometrial polyps,intrauterine residual,submucous myoma and endometrial cancer.The etiology and pathogenesis of different diseases are different,and the difference of early clinical manifestations is not obvious,which increases the difficulty of clinical diagnosis and treatment.As the most accepted ultrasound examination,if we can do our best to provide clear ultrasound image and effective diagnosis information for patients in the diagnosis of intrauterine lesions,we can reduce the number of patients’ examination,reduce the social cost of examination as much as possible,reduce the pain of patients’ examination,and protect patients’ uterus as much as possible.Therefore,we should strengthen the research of ultrasound to provide a method for the diagnosis of intrauterine lesions.This method can provide more information for the diagnosis of patients and realize the physical and mental protection of patients.Objective:To investigate the diagnostic effect and application value of two-dimensional ultrasound combined with Doppler ultrasound in patients with intrauterine lesions,and to provide evidence and reference for clinical diagnosis and treatment.Methods: A total of 300 patients with uterine cavity lesions admitted to Xinxiang Municipal Government Hospital and Xinxiang City First People’s Hospital from May 1st to May 20 th,2019 were selected.All patients were re-improved after admission.Ultrasound,Doppler ultrasonography,gynecologic surgery for patients with suspected lesions and patients with surgical indications,pathological examination(gold standard)during operation,two-dimensional ultrasound combined with Doppler ultrasonography results and surgical tissue examination results Comparison,analysis of intrauterine lesion detection rate,pathological diagnosis coincidence rate,complete statistical analysis;surgical records as a gold standard,compare the accuracy of two-dimensional ultrasound examination,Doppler ultrasound examination;analysis of two-dimensional ultrasound combined Diagnosing rate,missed diagnosis rate,misdiagnosis rate of intrauterine lesions in Puy’s ultrasound;analyze the diagnostic efficacy of two-dimensional ultrasound combined with Doppler ultrasound in patients with intrauterine lesions(including:diagnosis Sensitivity,specificity.Results:1.All the 300 patients completed two-dimensional ultrasound diagnosis,214 cases were diagnosed as intrauterine lesions,200 cases were confirmed as intrauterine lesions by pathological examination,and 14 cases did not have obvious abnormalities.Among 86 patients without intrauterine lesions,17 had no endometrial lesions and 69 had endometrial lesions.The sensitivity,specificity,positive predictive value and negative predictive value were 74.35%,54.84%,82.99% and 19.77%,respectively.,.2.All 300 patients were diagnosed by Doppler ultrasound,including 220 cases of endometrial lesions,204 cases of endometrial lesions confirmed by pathological examination,16 cases of normal.Among 80 cases without intrauterine lesions,65 cases had endometrial lesions and 15 cases had no intrauterine lesions.The sensitivity,specificity,positive predictive value and negative predictive value of Doppler ultrasound were 75.84%,48.39%,92.73% and 18.75% respectively.3.All 300 patients completed two-dimensional ultrasound combined with Doppler ultrasound,including 256 cases of endometrial lesions,247 cases of endometrial lesions confirmed by pathological examination,9 cases of normal.Among 44 cases without intrauterine lesions,22 cases with endometrial lesions and 22 cases without intrauterine lesions were diagnosed pathologically.The sensitivity,specificity,positive predictive value and negative predictive value were 91.82%,70.97%,96.48%and 4.55% respectively.4.All 300 patients completed two-dimensional ultrasound,two-dimensional ultrasound combined with Doppler ultrasound,and compared the results with pathological results.The diagnosis rate of two-dimensional ultrasound was 72.33%,and that of Doppler ultrasound was 89.67%.The diagnosis rate of two-dimensional ultrasound combined with Doppler ultrasound was higher than that of two-dimensional ultrasound(P < 0.05).5.All the 300 patients completed the diagnosis of Doppler ultrasound,two-dimensional ultrasound and Doppler ultrasound,and compared the diagnosis results with pathological results.The diagnosis rate of Doppler ultrasound was 73.0%,that of two-dimensional ultrasound combined with Doppler ultrasound was 89.67%,and that of two-dimensional ultrasound combined with Doppler ultrasound was higher than that of Doppler ultrasound(P < 0.05).6.There were significant differences in sensitivity,specificity,positive predictive value and negative predictive value between two-dimensional ultrasound and two-dimensional ultrasound combined with Doppler ultrasound(P < 0.05).7.The diagnostic sensitivity,specificity and negative predictive value of Doppler ultrasound combined with two-dimensional ultrasound were statistically significant(P < 0.05);the negative predictive value of two-dimensional ultrasound combined with two-dimensional ultrasound combined with Doppler ultrasound was not statistically significant(P > 0.05).Conclusion: two-dimensional ultrasound combined with Doppler ultrasound is simple,economical,noninvasive and effective in the diagnosis of intrauterine diseases.It can obtain high diagnostic sensitivity and specificity,help to guide clinical treatment,and is worth popularizing. |
