| Objective: In this study,the elderly falls and fractures with orthostatic hypotension in patients were analyzed,aims to address orthostatic hypotension in patients with fracture of elderly fall detection rate,awareness,and understanding of the risk factors associated with orthostatic hypotension,to guide the elderly fracture caused by falls prevention provide data and theoretical basis,reduce the occurrence of falls and fractures.Methods: Study of 194 elderly inpatients with falls and fractures hospitalized in the department of orthopedics and spine surgery of First Hospital of Qinhuangdao from October 2020 to December 2021,measuring the recumbent position and standing blood pressure,heart rate,and collect the patients’ medical history,medication history,living habits,assay index,echocardiography and other related information,record data and statistical analysis.Result: 1.The detection rate of orthostatic hypotension in elderly patients with falls and fractures was 27.3%(53 cases /194 cases);The awareness rate of orthostatic hypotension was only 2.6% in all patients.There was no statistical difference in gender between fall and fracture group and fall and fracture combined with orthostatic hypotension group(P > 0.05).2.The proportion of 60~69 years old in the group of fall and fracture combined with orthostatic hypotension was 18.6%(19cases / 102 cases),The proportion of 70~79 years old in the group of fall and fracture combined with orthostatic hypotension was 34.6%(26 cases /75 cases),The proportion of 60~69 years old in the group of fall and fracture combined with orthostatic hypotension was 47.1%(8 cases /17cases),the differences was statistically significant(P < 0.05).3.Blood pressure and heart rate in decubitus and orthostatic position were compared between the two groups,and the differences was statistically significant(P< 0.05);There was no significant difference in body mass index and related laboratory test indexes between the two groups(P > 0.05).4.After the comparison between the fall and fracture group with orthostatic hypotension and the simple fall and fracture group,binary multiple factor Logistic regression analysis was performed for statistically of variables with significant differences under the standard of P < 0.05.It showed that age,decubitus systolic and diastolic blood pressure,decubitus heart rate,hypertension,diabetes mellitus and cerebral apoplexy were correlated with orthostatic hypotension combined with falls and fractures(P < 0.05).Conclusion: The detection rate of orthostatic hypotension in elderly patients with falls and fractures was 27.3%,and the awareness rate was only 2.6%.The incidence of orthostatic hypotension increased with age.We can monitor the blood pressure and heart rate of the elderly in supine and standing position,and timely detect the presence of orthostatic hypotension,so that early detection and early prevention and treatment measures can be taken.Orthostatic hypotension increased in patients with hypertension,diabetes,and cerebral apoplexy,which are risk factors for falls and fractures combined with orthostatic hypotension. |
