Abstract/Summary:
ObjectiveTo analyze the clinicopathological characteristics and prognostic factors of lung adenocarcinoma and lung squamous carcinoma,and to construct nomograms that effectively predict the 3-year and 5-year survival rates of patients with lung adenocarcinoma and lung squamous carcinoma.MethodsLung adenocarcinoma and squamous lung cancer cases from 2010 to 2015 were extracted in the surveillance,epidemiology and end results(SEER)database.First,the clinicopathological characteristics and overall survival(OS)of lung adenocarcinoma and lung squamous carcinoma were analyzed using chi-square test,rank sum test and KaplanMeier curve.Then,independent factors affecting OS of lung adenocarcinoma and lung squamous carcinoma were established by univariate and multifactorial analyses.Then,Nomograms for predicting 3-year and 5-year survival rates in patients with lung adenocarcinoma and lung squamous carcinoma were constructed using R language.Finally,the performance of Nomogram was evaluated by consistency index(C-index),receiver operating characteristic(ROC)curve,area under the curve(AUC),calibration curve and decision curve analysis(DCA).ResultsA total of 27,651 cases of adenocarcinoma of the lung and 17,817 cases of squamous lung cancer were included in the analysis.Lung adenocarcinoma and lung squamous carcinoma were significantly different in terms of demographic characteristics(sex,age,race and marital status),disease characteristics(tumor laterality,tumor location,histologic grade,tumor stage,T stage,N stage and M stage),treatment(surgery and chemotherapy)and OS(all P<0.01).Adenocarcinoma of the lung has a better OS than squamous lung cancer.Independent factors influencing OS in patients with lung adenocarcinoma were gender,age,race,marital status,tumor location,histologic grade,T-stage,N-stage,Mstage,surgery,and chemotherapy.The C-index of lung adenocarcinoma nomogram was0.776(95% CI: 0.773-0.779),with a calibration curve slope close to 1.It performed better than TNM staging in terms of ROC curve,AUC value and DCA curve.Independent factors affecting OS in patients with squamous lung cancer were gender,age,race,marital status,tumor location,T stage,N stage,M stage,surgery,and chemotherapy.The C-index of nomogram for squamous lung cancer was 0.737(95% CI: 0.733-0.741),with a calibration curve slope close to 1.It performed better than TNM staging in terms of ROC curve,AUC value and DCA curve.Conclusion(1)Adenocarcinoma of the lung and squamous lung cancer have different clinicopathological features and prognostic factors,and adenocarcinoma of the lung has a better OS than squamous lung cancer.(2)The nomogram constructed in this study can accurately predict the 3-and 5-year survival rates of patients with lung adenocarcinoma and squamous lung cancer,which can help clinicians to effectively assess the disease as well as make treatment decisions.
