Objective In this study, dynamic 18F-FDG PET/CT(Fludeoxyglucose Positron Emission Tomography/Computed Tomography) imaging was used to examine patients with high suspicion of lung cancer at the first diagnosis. The diagnostic efficacy of dynamic 18F-FDG PET/CT imaging in lung cancer and related lymph nodes was evaluated by comparing the Metabolic Rate of FDG(MRFDG) of dynamic imaging and the SUVmax (Standard Uptake Value) of static imaging.Material and Methods This study recruited patients who were first diagnosed as highly suspected of lung cancer in Zhongda Hospital Affiliated to Southeast University for dynamic 18F-FDG PET/CT imaging. The Patlak model was constructed using the obtained images and the parameters such as MRFDG were calculated. The K-S test was used to compare the radioactivity concentration curves of benign and malignant lesions in dynamic images. The Wilcoxon rank sum test was used to compare the MRFDG of benign and malignant lesions in dynamic images between groups. The receiver operating characteristic (ROC) curve was used to compare the direct diagnostic efficacy and image diagnostic efficacy of static 18F-FDG PET/CT imaging and dynamic 18F-FDG PET/CT imaging for malignant lesions. Results A total of 36 patients were included in this experiment,37 lung lesions were successfully delineated, and 15 lymph node images were collected. There were significant differences in the radioactivity concentration curves of benign and malignant lesions in the dynamic images of primary lung lesions and lymph nodes. The radioactivity concentration of malignant lung lesions increased with time, while benign lesions showed the opposite trend. There were significant differences in the MRFDG of dynamic images between benign and malignant primary lung lesions. However,no difference was found between benign and malignant lymph nodes. There was no significant difference between dynamic images and static images in the direct diagnostic performance of primary lung lesions and lymph nodes. In terms of image diagnostic efficiency, dynamic images of pulmonary primary lesions have more advantages than static images, and the difference between the two is statistically significant. Conclusion The MRFDG of dynamic 18F-FDG PET/CT imaging can be used to identify the pathological properties of primary lung lesions. And the image diagnostic performance of dynamic 18F-FDG PET/CT imaging is better than that of static 18F-FDG PET/CT imaging.Dynamic 18F-FDG PET/CT imaging has certain advantages in the early diagnosis of primary lung cancer.
