Objective:By collecting the general data and blood test indicators of children with pneumonia,pneumonia with wheezing and asthma group,and comparing the relevant indicators of children,the correlation between pneumonia and pneumonia with wheezing and the clinical significance of predicting the related indicators of wheezing and asthma were analyzed,so as to guide the clinical prevention of pneumonia with wheezing and effective intervention on the risk factors that may progress into asthma.Methods:A total of 120 children with pneumonia(pneumonia group),153 children with pneumonia with wheezing(wheezing group),and 73 children with asthma(asthma group)who met the inclusion and exclusion criteria in this study were collected from the Affiliated Hospital of Yan ’an University from October 2021 to December 2022.Blood test indicators,relevant cell ratios and general data among the three groups were analyzed,meaningful indicators were included in the multi-factor analysis,and suspicious influencing factors were screened for ROC curve analysis,so as to clarify the critical value between relevant indicators and predicted diseases.Result:1.There were statistically significant differences in BMI,WBC,NLR,PLR,SII,EOS,RDW,platelet ratio and SAA between pneumonia group and wheezing group(P<0.05).There was no significant difference in MPV,CRP and PCT(P>0.05).The proportion of fever and mycoplasma positive in pneumonia group was higher than that in wheezing group,and the proportion of virus positive in wheezing group was higher than that in pneumonia group.2.Spearman Spearman correlation analysis was conducted with whether children with pneumonia were accompanied by wheezing as the dependent variable,and possible related clinical indicators as independent variables.The results were as follows: NLR(rs=-0.426,P<0.001),PLR(rs=-0.277,P<0.001),SII(rs=-0.320,P<0.001),SAA(rs=-0.362,P<0.001),fever(rs=-0.667,P<0.001),mycoplasma infection(rs=-0.487,P<0.001)were negatively correlated with asthma in children with pneumonia.Gender(rs= 0.121,P=0.047),BMI(rs= 0.248,P<0.001),WBC(rs= 0.202,P=0.001),EOS(rs=0.152,P=0.012),RDW(rs= 0.153,P=0.011),platelet ratio(rs= 0.432,P<0.001)and viral infection(rs= 0.136,P=0.024)were positively correlated.3.Fever,thrombocytosis and mycoplasma infection were independent influencing factors of pneumonia accompanied by wheezing,and the accuracy of predicting pneumonia accompanied by wheezing was 84.2%>60%,with good accuracy.ROC curve analysis was performed on platelet ratio and joint predictor.The area under curve of platelet ratio and joint predictor was 0.751 and 0.909,sensitivity was 57.5% and 77.8%,specificity was 79.2% and 93.3%,Yodon index was 0.367 and 0.711,and the critical value was 0.325 and 0.107.4.Single factor analysis was performed among the pneumonia group,asthmatic group and asthma group,and it was found that there was statistical significance in BMI,WBC,NLR,PLR,SII,EOS,RDW,MPV,platelet ratio,CRP,SAA,PCT,mycoplasma infection and viral infection in the three groups(P<0.05).The above indexes were included in the multi-factor analysis,and it was found that mycoplasma infection positive was more common in the pneumonia group.The platelet ratio of asthma group and asthmatic group was higher than that of pneumonia group.The PCT of asthmatic group and pneumonia group was higher than that of asthmatic group,and the EOS was lower than that of asthmatic group.PLR and NLR in pneumonia group were higher than those in wheezing group.NLR in asthma group was higher than that in pneumonia group.The MPV of asthma group was higher than that of asthmatic group.Other indicators such as BMI,WBC,SII,RDW,CRP,SAA and other factors had no difference between the three groups.5.ROC curve analysis was performed on factors with predictive value in the wheezing group and asthma group,and the areas under the curve of platelet ratio,NLR,MPV and PLR were 0.435,0.801,0.664 and 0.727,respectively,with sensitivity of 4.1%,78.1%,84.9% and 61.6%.The specificity was 98.0%,73.9%,45.1%,79.7%,the Yoden index was 0.021,0.52,0.3,0.413,and the critical value was 0.635,1.16,9.75,125.99.Conclusion:1.The inflammatory index and ratio,the proportion of fever and mycoplasma infection in the pneumonia group were significantly higher than those in the asthma group.The ratio of inflammatory cells such as PLR and NLR in the asthma group were higher than those in the pneumonia group and the asthma group,and the proportion of viral infection in the asthma group was higher than that in the pneumonia group and the asthma group.2.Platelet ratio and combined predictors can be used as influencing factors of pneumonia with wheezing.Platelet ratio >0.325 or combined predictors >0.711 can be used as one of the predictors of pneumonia with wheezing.3.NLR and PLR can be used as influencing factors for predicting asthma with pneumonia accompanied by wheezing.When NLR>1.16 and PLR>125.99,they can be used as one of the predictors for the possible development of asthma with pneumonia accompanied by wheezing.
