ObjectiveThis study investigated the effect of cataract ultrasound aspiration combined with capsular tension ring(CTR)implantation on the postoperative refractive status of patients with high myopia combined with cataract.MethodA prospective randomized controlled trial design was used.Sixty-eight eyes of46 patients with high myopia combined with cataract treated at the Nanchang University Eye Hospital between May 2022 and October 2022 were included in the study,and were divided into experimental and control groups using a randomized number table method,with 34 patients and 34 eyes in each group.The experimental group underwent cataract ultrasound aspiration + IOL(intraocular lens)+ capsular bag tension ring implantation,while the control group underwent cataract ultrasound aspiration + IOL implantation.Patients in both groups were monitored for best corrected visual acuity(BCVA),anterior chamber depth(ACD),IOL tilt,IOL eccentricity,refraction,refractive error,and refractive drift at preoperative and postoperative follow-up at 1 day,1 week,1 month,and 3 months.ResultThere was no statistically significant difference(P>0.05)in the best corrected visual acuity(BCVA)comparison between the two groups on preoperative and postoperative day 1,week 1,month 1,and month 3,and the postoperative visual acuity was better than the preoperative visual acuity(P<0.001);there was no statistically significant difference(P>0.05)in the anterior chamber depth(ACD)comparison between the two groups on preoperative and postoperative day 1,week 1,month 1,and month 3,and the postoperative anterior chamber was deeper than the preoperative anterior chamber(P<0.001);IOL level,vertical and total tilt continued to increase in both groups at 1 day,1 week,1 month and 3 months after surgery,and the difference was statistically significant(P<0.05);IOL level,vertical and total tilt in the experimental group was smaller than that in the control group at 3 months after surgery(t=2.904,P<0.05;t=7.229,P<0.001;t=7.00,P< 0.001);IOL horizontal,vertical and eccentricity continued to increase in both groups at 1 day,1 week,1month and 3 months postoperatively,with statistically significant differences(P<0.05);vertical and total eccentricity in the experimental group was smaller than that in the control group at 1 month and 3 months postoperatively(t=6.358,P<0.001;t=2.267,P=0.027;t=5.261,P<0.001;t= 2.525,P=0.015);the differences in refractive error and refractive error between the two groups at 1 day,1 week,1 month and 3months after surgery were not statistically significant(all P>0.05);the differences in refractive drift between the two groups from 1 day to 1 week and from 1 week to 1month after surgery were not statistically significant(P>0.05): the refractive drift in the experimental group from 1 month to 3 months after surgery was less than that in the control group(P<0.05);the differences in refractive error between the two groups from 1 day to 1 week,from 1 week to 1 month and from 1 day to 1 month after surgery were not statistically significant(P>0.05).The amount of refractive drift was highly positively correlated with the change in anterior chamber depth in both groups from 1 day to 1 week,1 week to 1 month,and 1 month to 3 months after surgery(P <0.001).ConclusionFor cataract surgery combined with CTR implantation in patients with high myopia,although it cannot reduce the postoperative refractive error,it can reduce the amount of IOL tilt and eccentricity,reduce anterior chamber fluctuations,stabilize postoperative diopters,and reduce refractive drift.
