| Objective:This study aimed to assess the overall level of the fear of cancer recurrence(FCR)among hospitalized breast cancer patients,to discuss the influencing factors of FCR.At the same time,the intervention program of transcendental meditation for breast cancer was constructed to verify further its effect on the level of fear,anxiety,depression and social constraint in patients with breast cancer to improve the quality of life,and for the formulation of relevant measures to provide a reference basis.Methods:(1)Cross-sectional survey:purposive sampling selected 219 breast cancer patients in breast surgery and oncology departments of one Triple-A hospital in Jiangxi province from April to August 2022.The investigation tools included:a general data questionnaire,a fear of progression questionnaire short form(FoP-QSF),hospital anxiety and depression scale(HADS),social constraints scale(SCS).Descriptive statistics,One-way ANOVA,and multiple linear regression analysis with SPSS 26.0 software were used to study the current level of FCR in patients with breast cancer and its influencing factors.At the same time,correlation analysis was used to analyze the correlation between the scores of FoP-Q-SF,HADS,and SCS scales in patients with breast cancer.(2)Stage of empirical research on intervention programs: Based on the results of systematic literature review and current investigation,according to the treatment characteristics of breast cancer,the intervention program of transcendental meditation was constructed through expert demonstration and experimental team discussion.The patients from the breast surgery department of one Triple-A hospital in Jiangxi province were selected as experimental subjects from August to December 2022 by the method of quasi-experimental study;they were randomly divided into the control group and the observing group according to the lottery of disease area,40 cases in each group.The control group was given routine nursing measures,and the observing group received the intervention of transcendental meditation program.Before and after intervention,FoP-Q-SF,HADS and SCS scales were used to compare the effects of patients.Results:(1)Current investigation results:(1)In this study,the total score of FoP-Q-SF scale in breast cancer patients was(35.56±3.38)points,61.2% of which reached the clinical level(≥34 points).The total score of HADS scale was(14.62±4.33)points,among which 42.1% and 47.5% had mild anxiety and depression symptoms,and the score of SCS scale was(33.86±6.88)points.(2)Results of One-way ANOVA: In the general data,there were statistical differences in the total score of the FoP-Q-SF scale among breast cancer patients in terms of age,marriage,degree of education,occupation,LAINCOME,payment method,diagnosis time,mode of operation and underlying diseases(P<0.05).At the same time,through Post Hoctests,it was found that the total score of FoP-Q-SF scale among breast cancer patients with LAINCOME <1000 yuan was significantly higher than that in other income groups(P<0.05);In terms of occupation,the total score of FoP-Q-SF scale in unemployed patients was higher than that in other working and retired patients(P<0.05).The total score of the FoP-Q-SF scale in patients diagnosed from 1 to 6 months was significantly higher than that in other patients(P<0.05).(3)Results of correlation analysis: the FCR was positively correlated with anxiety,depression and social constraints(r=0.522~0.882,P<0.01),and the score of physical health,social family dimension were also positively correlated with anxiety,depression and social constraints.(4)Results of multiple linear regression: social constraints,age,marriage,anxiety symptoms,occupation,LAINCOME entered the regression equation successively(adjusted R~2=0.757,P<0.001).(2)Results of empirical study: One case dropped out in the experimental group,and finally 79 patients completed the intervention(39 cases in the experimental group and 40 cases in the control group).Before the experiment,the baseline data of the two groups were compared evenly,and the results showed no statistical significance(P>0.05),which was comparable.At the same time,intragroup comparison of all indicators before and after experiment of the two groups showed that,compared with before the experiment,the control group had no statistical criteria in total score of FoP-Q-SF,HADS and SCS scales and all dimensions(P>0.05).while the total scores of FoP-Q-SF,HADS and SCS scales in the experimental group were significantly reduced(P<0.05).The inter-group comparison of various indicators between the two groups showed that compared with the control group,the scores of HADS and SCS scales and various dimensions of the fear of cancer recurrence in the experimental group had marked criteria after intervention(P<0.05).Conclusion:1.The incidence of psychological dysfunction of FCR in breast cancer patients is high,which is not optimistic.The FCR in breast cancer patients is significantly correlated with anxiety,depressive symptoms and perceived social constraints.In multiple linear regression analysis,age,marriage,occupation,LAINCOME,anxiety symptoms and social constraints all predicted the FCR,especially perceived social constraints.It is suggested that medical staff should assess and identify high-risk groups early,and explore effective strategies to reduce the fear of cancer recurrence,focusing on social cognition and psychological aspects.2.The transcendental meditation program can effectively mitigate the FCR in breast cancer patients,relieve anxiety and depression symptoms,and reduce the level of social constraints,which is worthy of reference and promotion in clinical cancer mental health management. |
