Objective:Exploring the correlation between serum NLR(neutrophil/lymphocyte ratio)and UA(uric acid)levels and mild cognitive impairment,in order to identify indicators for early detection of cognitive impairment.Materials and methods:From November 2016 to January 2023,64 patients with Alzheimer’s disease(AD group)and 54 patients with mild cognitive impairment(MCI group)who were hospitalized in the Department of Geriatrics,General Practitioner’s Medicine,and Neurology of the First Affiliated Hospital of Nanchang University were collected as cognitive impairment groups,and 66 healthy controls(HC group)who matched baseline data during the same period were randomly collected.All patients were evaluated for cognitive function using various neuropsychological tests,At the same time,the baseline data and auxiliary examinations of the subjects were collected to detect the serum neutrophil lymphocyte ratio and uric acid level,analyze the differences in NLR and UA levels among the groups,use correlation analysis to evaluate the correlation between serum NLR,uric acid levels and MMSE scores,and evaluate the diagnostic efficacy of serum NLR and uric acid levels on MCI and AD by drawing ROC curves.Area under curve(AUC)was used for comparison.Multivariate logistic regression analysis was performed to analyze the risk factors affecting MCI.Result:1.The serum NLR value of the cognitive impairment group was significantly higher than that of the HC group(p<0.05),and the UA value was significantly higher than that of the HC group(p<0.05);The serum NLR value in the AD group was higher than that in the MCI group(p<0.05),while the serum UA value in the AD group was lower than that in the MCI group(p<0.05).2.The serum NLR level in the cognitive impairment group was negatively correlated with MMSE score,while the serum UA level was positively correlated with MMSE score(p<0.001).3.Using ROC analysis data,it was found that the area under the curve of NLR for distinguishing between HC and MCI groups was 0.718(p<0.001,95% CI:0.627-0.809),with an optimal cutoff value of 1.99;The area under the curve used to distinguish between the HC group and the AD group is 0.813(p<0.001,95% CI:0.729-0.879),with an optimal cutoff value of 2.55.The area under the curve of UA used to distinguish between HC group and MCI group is 0.713(p<0.001,95% CI:0.598-0.788),and the optimal cutoff value is 328.45;The optimal cutoff value for distinguishing between the HC group and the AD group is 319.45,with an area under the curve of 0.804(p<0.001,95% CI: 0.742-0.884).4.The results of multivariate logistic regression analysis showed that after excluding confounding factors such as Hb and LDL,NLR(OR=3.99;95% CI:1.97-8.06,p<0.05)remained an independent risk factor for MCI.Conclusion:The serum NLR level in the cognitive impairment group was significantly higher than that in the HC group,while the serum UA level was lower than that in the HC group.There is a certain correlation between serum NLR and UA levels and cognitive function,and studying the changes in serum NLR and UA levels has certain reference value for the early diagnosis of cognitive dysfunction.
