Association Of Alcohol Consumption Behavior With Left Atrial Appendage Thrombus In Elderly Patients With Paroxysmal Atrial Fibrillation

Abstract/Summary:
Objective:To investigate the influence of drinking behavior on left atrial appendage thrombus in senile patients with paroxysmal atrial fibrillation,and to provide reference value for precise prevention and control of stroke in them.Methods:Elderly patients with paroxysmal atrial fibrillation who planned to undergo radiofrequency ablation in the Cardiovascular Department of Jiangxi Provincial People’s Hospital from January 2019 to July 2022 were selected.A total of 39 patients who met the inclusion and exclusion criteria and were clearly diagnosed with left atrial appendage thrombus by transesophageal echocardiography were included in the experimental group.Then,according to the principle of the same gender and similar age(age difference<5 years),117 patients without LAA thrombus admitted at the same period were selected as the control group with 1:3 pairing.General clinical data,alcohol-related information,hematological indexes and imaging results of the patients were collected.SPSS 26.0 software was used for multivariate logistic regression analysis of LAA thrombus and correlation analysis of drinking behavior with LAA thrombus.Results:1.Univariate analysis showed that there were statistically significant difference between the two groups in the presence of chronic heart failure,alcohol consumption prothrombin time,prothrombin time activity,international normalized ratio(INR),N-terminal fragment B-type natriuretic peptide(NT-proBNP),left atrial diameter and left ventricular ejection fraction(LVEF)(P<0.05).After adjusting for confounding factors,multivariate regression analysis found that just left atrial diameter and alcohol consumption were independently correlated with left atrial thrombus,and alcohol consumption was the most correlated with LAA thrombus(OR:4.262,95%CI:1.621-11.208,P=0.003);2.The proportion of alcohol drinkers in the thrombus group was significantly higher than that in the non-thrombus group(p=0.002),and the distribution of drinking type,drinking frequency,drinking degree and drinking duration were different between the two groups(P<0.05);3.The type,frequency,degree and duration of drinking were divided into dummy variables and logistic regression analysis showed that compared with non-drinkers,beer drinkers had an increased risk of LAA thrombus(OR:1.013,95%CI:1.002-1.365),liquor drinkers had a higher risk of LAA thrombus(OR:1.315,95%CI:1.109-2.742),while wine consumption had no statistically significant effect on LAA thrombus(P=0.685).The risk of LAA thrombosis gradually increased with the extension of drinking time or the increase of drinking frequency.However,compared with non-drinkers,light drinkers did not increase the risk of LAA thrombosis(P=0.436),while moderate and heavy drinkers significantly increased the risk of LAA thrombosis(OR of 1.122 and1.534,respectively).Conclusion:1.Drinking behavior is significantly correlated with LAA thrombus in elderly patients with PxAF;2.Moderate to heavy drinking is associated with a significantly increased risk of LAA thrombus,while mild drinking is not.The longer the drinking time and the more frequent the drinking,the higher the risk of LAA thrombus.3.For elderly patients with PxAF,liquor consumption may be an important risk factor for LAA thrombus,followed by beer,while wine consumption has no effect on the increased risk of LAA thrombus.
