| Objective:As a life-threatening cardiovascular emergency,fulminant myocarditis has a high fatality rate.The purpose of this paper is to investigate the efficacy of gamma globulin in different doses and timing,in order to provide a comparison of treatment plans for this disease.Methods:A total of 47 patients meeting the criteria were included and screened for retrospective analysis.Based on dose and timing,the trial was split into two groups.The dose grouping was divided into greater than 10 g/d(group A),less than or equal to 10 g/d(group B),and no treatment with gammaglobulin(group C).The time grouping was divided into no gammaglobulin use(group C),use within 24 hours of admission(group D),and use after 24 hours of admission(group E).Patients’ age,heart rate,and blood pressure at admission were compared;NT-pro BNP,CK-MB,ALT,creatinine,and LVEF before and after treatment;length of stay in hospital,and survival rate.Results:For dose grouping:1.Heart rate was higher in group A than in group B,and the difference had statistical significance(adjusted P=0.014,P<0.05).2.After treatment,BNP decreased significantly in group A patients,and the mean left ventricular ejection fraction improved by 13.9%,with a statistically significant difference(P<0.05);BNP decreased in group C after treatment,with a statistically significant difference(P=0.025,P<0.05).For time grouping:3.Patients in group C had a significant decrease in BNP after treatment,and the difference possessed statistical significance(P=0.025,P<0.05).The mean value of BNP in group D decreased by 10813.14 after treatment,and the difference was statistically significant(P<0.05);the left ventricular ejection fraction increased significantly after treatment compared with that before,and the difference was statistically significant(P=0.012,P<0.05).Conclusion:In dose,large dose(After 10 g/d)gamma globulin treatment of fulminant myocarditis,BNP level was significantly decreased and LVEF level was significantly increased;in terms of timing,early administration of gammaglobulin(within 24 hours of admission)also resulted in a significant reduction in BNP levels and some improvement in cardiac function. |
