Objective:To investigate the distribution characteristics of high-order aberration in the area with 4mm central corneal diameter in cataract patients,and to explore its correlation with age,corneal curvature,total corneal refractive power,astigmatism and corneal optical density count,so as to provide reference for the selection of refractive intraocular lens.Methods:This study is a retrospective case study.Cataract patients treated in Chang Sha Aier Eye Hospital from January 2021 to December 2021 were included as the research object,corneal pannus,corneal leukoplakia,pterygium,moderate to severe dry eyes,previous history of corneal surgery and anti-glaucoma surgery,as well as pterygium surgery were excluded.The same Pentacam anterior segment analyzer was used.Patient’s age,corneal curvature,total corneal refractive power,corneal low-order astigmatism,corneal optical density value and high-order aberration in the area with 4mm central diameter of cornea under the natural pupil were collected.According to the high-order aberration value of cornea,the patients were divided into three groups: < 0.3μm;0.3~0.5μm;> 0.5μm.SPSS25.0 software was used to analyze the data.Spearman correlation was used to analyze the correlation between the data and factors such as age,corneal astigmatism,corneal optical density count,corneal curvature and total corneal refractive power.The difference was statistically significant when p ≤ 0.05.Chi square test was used to analyze the correlation between gender and high-order aberration of cornea in the three groups.P ≤ 0.05 means the difference is statistically significant.Results:A total of 1754 patients with various types of cataract(2151 eyes)were included in this study.There were 715 males and 1039 females,aged from 19 to 96(mean 68.26 ± 11.38)years.The total high-order aberration ranged from 0.045 to 1.401 μm.The median was 0.179 μm;Where < 0.3 μm was 1795 eyes(83.4%),0.3 ~ 0.5 μm was 296 eyes(13.7%),> 0.5 μm was 60 eyes(2.8%);The simulated corneal surface curvature(SIM K)was 38.9 ~ 50.6D,and the median was 44.10d;The simulated corneal astigmatism(Sim K astig)was 0 ~ 5.9D,and the median was 0.70D;Total corneal refractive power keratometry(TCRP K)was 38.8 ~ 50.4D,with a median of 43.90D;Total corneal refractive power astigmatism(TCRP astig)was 0 ~ 6.2D,with a median of 0.80D;The median values of 0-2mm corneal optical density(COD),2-6mm corneal optical density and total corneal optical density(TCOD)were18.60GSU(14.5 ~ 42.3GSU)respectively;18.00GSU(14.1 ~ 34.4GSU)and 25.10GSU(15.2 ~ 42.3GSU).Chi square test showed that there was no correlation between gender and corneal high-order aberration in the three groups(P = 0.069);Spearman correlation analysis showed that the correlation between high-order aberration in the area with 4mm central corneal diameter and age,corneal curvature,astigmatism,COD in 0-2mm area,COD and TCOD in 2-6mm area was statistically significant(P <0.05).Corneal high-order aberration was significantly and moderately positively correlated with age(r = 0.510,P < 0.05),COD in 0-2mm area(r = 0.400,P < 0.05),COD in 2-6mm area(r = 0.585,P < 0.05)and TCOD(r = 0.555,P < 0.05);And Sim K(r = 0.192,P < 0.05),Sim K astig(r = 0.051,P < 0.05),TCRP K(r = 0.257,P < 0.05),TCRP astig(r =0.110,P < 0.05).Conclusion:In cataract patients,the high-order aberration in the area with 4mm central corneal diameter was < 0.3 μm accounts for 83.4%,0.3 ~ 0.5%μm accounts for 13.7%,> 0.5 μm accounts for 2.8%.Corneal high-order aberration was moderately correlated with age and corneal optical density;There was no correlation between corneal curvature and corneal low-order astigmatism.
