| Objective: To investigate the risk factors,clinical features and prognosis of first-time peritonitis in patients with continuous ambulatory peritoneal dialysis(CAPD),and to provide information for the prevention of peritoneal dialysis-associated peritonitis(PDAP)in patients with CAPD.Reference and reference,in order to improve and improve the quality of life and prognosis of patients with CAPD.Methods: From January 2018 to June 2022,184 patients who underwent CAPD in the peritoneal dialysis center of the Affiliated Hospital of Youjiang Medical College for Nationalities and received regular follow-up visits were selected as the research subjects in a cross-section.The general demographic information and clinical peritoneal dialysis information of the patients were collected.For relevant data,a self-made questionnaire was used to conduct a comprehensive investigation on patients’ life support,dressing change,follow-up visit frequency,employment status,personal monthly disposable income,bathing methods,etc.,and the collected CAPD patients were divided into observation group and control group.Group.Among them,the observation group was patients with CAPD who had peritonitis for the first time,and the control group was patients with CAPD who had never had peritonitis during the study period.For the comparison between the two groups,the measurement data conforming to the normal distribution adopts the mean ± standard deviation(),the measurement data without the normal distribution adopts the median or quartile(M±QR),the count data adopts the chi-square test,and the measurement data Data were compared between groups using two independent sample t-tests and non-parametric rank-sum tests.Risk factors were analyzed using univariate Logistic regression analysis models and binary Logistic regression analysis models.P<0.05 was used as the test level to evaluate and analyze CAPD patients.Risk factors for developing peritonitis.Results:(1)A total of 184 cases were included in this study,of which 84 cases developed PDAP,and the incidence rate was about 45.65%.);there was no statistical significance in terms of gender,age,ethnicity,education level,personal disposable monthly income,employment status,marital status,medical insurance,surgical method,and primary disease(P>0.05).(2)Among the 84 PDAP patients,21 cases(25.00%)occurred in spring,23 cases(27.38%)in summer,19 cases(22.62%)in autumn,and 21 cases(25.00%)in winter.(3)Among the 84 PDAP patients,46 cases were positive for bacterial culture,38 cases were not cultured,and the positive rate was54.76%.A total of 47 strains were cultured in positive patients,including 31 strains of Gram-positive bacteria(G+ bacteria).The proportion was 36.47%,and there were 16 strains of Gram-negative bacteria(G-bacteria),accounting for 18.82%.(4)Comparing the observation group with the control group,in white blood cells,neutrophils,lymphocytes,percentage of neutrophils,procalcitonin,erythrocyte sedimentation rate,prealbumin,fasting blood glucose,albumin,creatinine,creatinine clearance rate,There were statistically significant differences among blood calcium,blood potassium,blood phosphorus,and ferritin(P<0.05);There was no significant difference among protein,high-density lipoprotein and vitamin D3(P>0.05).(5)Incorporating the single factor Logistic regression analysis results into the binary Logistic regression analysis results showed that the differences in dialysis age,PCT,K,Ca,and PA were statistically significant(P<0.05),and the independent predictors were included in the ROC curve,the results showed that Dialysis age and PA are risk factors for PDAP in CAPD patients.(6)The questionnaire items were included in single-factor Logistic regression analysis,multi-factor Logistic regression,and ROC curve analysis.The results showed that gastrointestinal dysfunction,showering,less frequent follow-up visits,no dressing changes for a long time,and no regular replacement of external short tubes were Risk factors for PDAP in CAPD patients.Conclusions:(1)The incidence rate of PDAP in our center is high,and the pathogenic bacteria of PDAP are mainly Gram-positive bacteria;(2)The risk factors for the first occurrence of peritonitis in CAPD patients in our center are short dialysis duration,low anterior white Proteinemia,gastrointestinal dysfunction,showering,less frequent follow-up visits,no dressing changes for a long time,and no regular replacement of external short tubes. |
