Objectives To investigate the specific level and its influencing factors of family resilience in stroke patients with hemiplegia, further analyze the relationship among family resilience, family function, relative support, friend support and community support, and explore the mediating effect between family resilience and family function of the various support systems mentioned above. To provide a basis for carrying out targeted family anti-stress intervention programs for stroke patients with hemiplegia.MethodsA cross-sectional survey was adopted to choose families of stroke patients with hemiplegia who were stay in the Department of Encephalopathy of the Second Affiliated Hospital of Anhui University of Chinese Medicine from Feb.2022 to Oct.2022.Family Resilience Assessment Scale(FRAS-C),General information questionnaire,Family Concern Index Questionnaire(APGAR),Family Inventory of Resources for Management(FIRM),Multidimensional Scale of Perceived Social Support(PSSS),and Social Support Rating Scale were used to investigate their family resilience standard, demographic data, family concerning, kinsfolk support, friend support and social support. The Mean and standard deviation were used to describe the family resilience level of stroke patients with hemiplegia, and the influencing factors were determined by univariate analysis, multiple stepwise linear regression analysis and mediating effect analysis.Results1.The scores of family resilience were(94.71±7.70),which was at a medium and low level. The scores of family communication and solve matter, using social resources, maintaining a positive attitude are respectively are(68.75±5.20),(8.06±1.83),(17.89±1.96).The total score of Family Concern Index Questionnaire ranged from5 to 15 points, with an average of(9.46±2.27)points,the FIRM ranged from 3 to 12 points, with an average of(7.71±2.11)points,the PSSS ranged from 15 to 52 points, with an average of(35.96±5.53)points,the Social Support Rating Scale ranged from 13 to 38 points, with an average of(25.26±5.07)points.2.The results of univariate analysis of family resilience in stroke patients with hemiplegia shows that there are statistically significant difference between the education level of patients, patient monthly income, whether it is associated with other chronic diseases, frequency of incidence, degree of physical impairment, the main caregivers’ education level, average monthly income, chronic diseases, family bread-bearer status and perceived health status of the primary caregivers(P < 0.05).3.Conclusion Correlation analysis results: the family resilience, family communication and solve matter, use of social resources, hold a positive attitude were shown positive correlated with the total score of APGAR,FIRM,PSSS,and Social Support.4.Multiple linear regression analysis results shows that there are six variables into the regression equation, including: Whether the patient has other chronic diseases,The main caregivers’ education level, the total score of APGAR,FIRM,PSSS,and Social Support, accounting for 89.0% of the total variation.5.Mediation effect analysis shows that the FIRM has a mediating effect in the relationship between family resilience and Family Concern Index,PSSS has a mediating effect in the relationship between family resilience and Family Concern Index(P < 0.05),Social Support has a mediating effect in the relationship between family resilience and Family Concern Index as well.FIRM,PSSS and Social Support has a chain mediation effect between family resilience and Family Concern Index.Conclusions1.Family resilience of stroke patients with hemiplegia is at a medium low level;2.In addition to patients’ monthly income, whether they have other chronic diseases, the severity of physical disorders, and the education level of caregivers, factors derived from consanguinity network(family function),kinship network(kinsfolk’ support, friends’ support),and geographical network(social support)can affect the family resilience of stroke patients with hemiplegia. It is advised that medical staffs should pay more concern to the families with low income, low education level and serious disease,at the same time, focus on assessing the social networks at all levels of the families of patients, and encourage patients to obtain reliable and long-term available resources from the social network. From a macro perspective, the government and society should speed up the formulation of corresponding medical and health fitness and improve relevant basic service facilities to help families of such patients with hemiplegia effectively solve their difficulties.
