| Objective:To compare the demographic indicators,living habits and serological indicators between gout group and non gout group in patients with hyperuricemia,and analyze the influencing factors of gout occurrence,so as to provide reference for the management and lifestyle intervention of gout high-risk groupsMethods:Using the method of random sampling,we selected patients with hyperuricemia who were detected in the physical examination center of Cangzhou People’s Hospital from January 1,2019 to June 1,2022.With my consent,we conducted a questionnaire survey in the form of telephone return visit and collected physical examination data in the hospital system.A total of1200 people were interviewed,1040 people were counted as effective,and the effective recovery rate was 86.7%.The questionnaire includes basic demographic data,behavior habits,etc.The patients were divided into two groups according to whether they had gout,namely gout group and non gout group,and the clinical and serological data were retrospectively analyzed.Statistical analysis was conducted with spss 28.0 software,single factor analysis was conducted with t-test and chi square test,and multivariate analysis was conducted with binary Logistic regression analysis.Results:1.General data:Among 1040 patients,882 were male(84.8%),158 were female(15.2%),and the gender ratio was 5.58:1.The number of cases in gout group and non gout group was 128 and 912 respectively.The incidence of gout in patients with hyperuricemia was 12.3%,and that in gout group and non gout group was 1:7.125.2.The survey results show that gender,education level,BMI,marital status,smoking history,drinking history,meat and vegetable diet,diet taste,and whether or not you often exercise will all affect gout attacks,and the levels of alanine aminotransferase,glutamic oxaloacetic transaminase,creatinine,and homocysteine in serological indicators will affect gout attacks.3.The binary logistic regression analysis showed that the risk of gout attack in women was 2.3% of that in men,that is,men were more prone to it;The risk of gout attack in the range of BMI from 24 to 27.9 and ≥ 28 was7.994 times and 4.542 times higher than that in the range of BMI ≤23.9,respectively;Unmarried,divorced,widowed or others increased the risk of gout attack by 35.899 times and 74.631 times compared with married/cohabiting respectively;The risk of gout attack in patients with alcohol/abstinence was 1.488 times higher than that in patients without alcohol;In dietary habits,the risk of gout attack with moderate taste and light taste was 7.5% and 7.9% of salty taste,respectively,that is,the risk of gout attack with salty taste was higher than that with moderate taste and light taste;The risk of gout attack increased by 0.01 times for every unit of increase in the level of alanine aminotransferase;The risk of gout attack increased 0.037 times with the increase of glutamic oxaloacetic transaminase level by one unit;The risk of gout attack increased 0.035 times with the increase of creatinine level by one unit;The risk of gout attack increased 0.027 times for every unit of increase in homocysteine level.4.The ROC curve analysis shows that the critical value of alanine aminotransferase,glutamic oxaloacetic transaminase and creatinine is29.35U/L,26.35U/L and 86.1 respectively for men,patients with higher BMI,drinking alcohol μ The critical value of mol/L and homocysteine is13.15 μ Mol/L,the greater the risk of gout attack.In addition,compared with the single index,the risk of gout attack caused by five factors including combined sex+BMI+drinking history+alanine aminotransferase+glutamic oxaloacetic aminotransferase+creatinine+homocysteine was higher(P<0.001),the area under the curve was 0.796,the sensitivity was 0.805,the specificity was 0.658,and the 95%CI was 0.756-0.835.Conclusions:1.The attack rate of gout in the physical examination center of Cangzhou People’s Hospital in recent three years accounted for 12.3% of the patients with hyperuricemia.Among the patients with hyperuricemia,the ratio of gout attack to no gout attack was 1:7.125.2.Male,higher body mass index,unmarried,divorced,widowed or other marital status,drinking history,and salty diet are all risk factors for gout attack.3.If the level of alanine aminotransferase is greater than 29.35U/L,the level of glutamic oxaloacetic aminotransferase is greater than 26.35U/L,the level of creatinine is greater than 86.1μmol/L,and the level of homocysteine is greater than 13.15μmol/L,the greater the risk of gout. |
