1.To investigate the role of spleen-mediated inflammation in early neurological deficits in AIS;2.To explore the correlation between spleen-mediated inflammation and the onset and severity of acute ACI.Objective: A total of 67 AIS patients within 24 hours of onset admitted to the Second Affiliated Hospital of Anhui Medical University from January 2022 to June 2022 were selected as the study group,including 49 patients with ACI,and 30 healthy physical examination subjects with similar age and gender to the study group were selected as the control group.The general clinical data,biochemical indicators,serum inflammatory factors and spleen ultrasound imaging indicators of all enrolled patients were collected.All the enrolled patients were divided into mild group(NIHSS<6points)and moderate to severe group(NIHSS≥6 points)according to the NIHSS score.The clinical data,blood biochemical indexes,serum inflammatory factors and the standardized difference of spleen volume within 24 hours of onset and 48 hours of onset of AIS in the two groups were analyzed.Receiver operating characteristic(ROC)curve was used to analyze the predictive value of serum inflammatory factors and standardized difference of spleen volume for the severity of AIS.In addition,the ACI group was compared with the control group,and the differences in clinical data,blood biochemical indicators,serum inflammatory factors and spleen volume between the two groups were analyzed.The receiver operating characteristic(ROC)curve was used to analyze the predictive value of serum IL-6,IL-12P70 and spleen volume standardized difference value for the onset of acute ACI.Spearman correlation test was used to analyze the correlation between serum IL-6,spleen volume IL-5,IL-6,IL-1β,IL-8,IL-17,IL-12P70,TNF-α,NLR and the standardized difference of spleen volume in the moderate and severe AIS group were higher than those in the mild AIS group,and the difference was statistically significant(P<0.05).2.The area under the curve(AUC)of serum IL-8,IL-17,TNF-α,IL-1β,IL-5 and NLR for predicting the severity of AIS was smaller than that of serum IL-6,IL-12P70 levels and spleen volume standardized difference.The area under the curve(AUC)of serum IL-6 for predicting acute AIS was 0.714(95%CI:0.591-0.838,P=0.003),and the best diagnostic value was 5.895pg/ml.The area under the curve(AUC)of serum IL-12P70 for predicting AIS was 0.719(95%CI:0.585-0.845,P=0.002),and the best diagnostic value was 0.650pg/ml.The area under the curve(AUC)of spleen volume standardized difference for predicting AIS was0.774(95%CI: 0.654-0.895,P=0.000),and the best diagnostic value was 0.058.3.Compared with the control group,the serum IL-6,IL-12P70 and spleen volume standardized difference were significantly increased in acute ACI patients(P<0.05).4.The area under the curve(AUC)of serum IL-6 for predicting acute ACI was 0.675(95%CI: 0.557-0.793,P=0.009),and the best diagnostic value was 3.27pg/ml.The area under the curve(AUC)of serum IL-12P70 for predicting acute ACI was 0.660(95%CI:0.538-0.781,P=0.018),and the best diagnostic value was 0.785pg/ml.The area under the curve(AUC)of spleen volume standardized difference for predicting acute ACI was 0.727(95%CI: 0.608-0.846,P=0.01),and the best diagnostic value was-0.003.5.The standardized difference of serum IL-6 and spleen volume in acute ACI was positively correlated with the degree of early neurological impairment(r=0.421,P=0.003,r=0.412,P=0.003)Conclusion: 1.In patients with acute ischemic infarction,the more severe neurological impairment,the higher the normalized difference of serum IL-5,IL-6,IL-1β,IL-8,IL-17,IL-12P70,TNF-α,NLR and spleen volume;2.The normalized difference of serum IL-6 and IL-12P70 levels and spleen volume can predict the severity of acute ischemic infarction.3.The standardized differences of serum IL-6,IL-12P70 and spleen volume were significantly increased in patients with acute anterior circulation stroke,and the more severe neurological impairment was,the higher the standardized differences of serum IL-6 and spleen volume were.
