Objective: Troponin T1(TNNT1)is highly expressed in a variety of cancer tissues,affects the proliferation and migration of tumor cells,and is associated with the survival and prognosis of patients.However,the correlation has not been studied in lung cancer.In this study,gene expression profile data and clinical data of non-small cell lung cancer(NSCLC)in The Cancer Genome Atlas(TCGA)database were mined using an online analysis website.To investigate the expression of TNNT1 mRNA in non-small cell lung cancer and analyze the correlation with prognosis.Protein expression of TNNT1 in advanced NSCLC was assessed by Immunohistochemical staining(IHC)and its relationship to clinicopathological parameters and overall survival and progression-free survival was analyzed.Enzyme linked immunosorbent assay(ELISA)was used to detect the expression of TNNT1 protein in the serum of NSCLC,and to investigate its diagnostic efficacy and prognostic value in NSCLC.Methods: 1.Log in to UALCAN(http: //ualcan.path.uab.edu/)on the online analysis website,select the option of "TCGA" database,use "TNNT1" as the keyword for search and analysis,and derive the expression profile and survival curve of TNNT1 mRNA in LUAD and LUSC.To investigate the expression of TNNT1 in NSCLC and its relationship with prognosis.Eighty histological specimens of patients with advanced NSCLC admitted to our hospital from January 2018 to September 2019 were selected.Immunohistochemical staining was used to detect the expression of TNNT1 protein in pathological samples and normal lung tissues of the same period,and the relationship between TNNT1 protein and clinicopathological parameters was analyzed.Kaplan-Meier survival curve and COX regression model were established to analyze the correlation between TNNT1 expression level and overall survival and progression-free survival.2.Peripheral blood serum samples of 70 NSCLC patients admitted to the Department of Respiratory and Critical Care Medicine of our hospital from June 2021to June 2022 were collected,and peripheral blood samples of 50 healthy people admitted to our hospital for physical examination during the same period were used as controls.Double antibody one-step sandwich enzymo-linked immunosorbent assay(ELISA)was used to detect the expression of TNNT1 protein in peripheral blood,analyze the relationship between TNNT1 expression and clinicopathologic data of patients,and establish the receiver operating characteristic curve,combined with the expression of CEA and CYFRA21-1 in serum of patients,to determine the diagnostic efficacy of TNNT1 combined with single and multi-index detection in NSCLC.Results: 1.The results of TCGA database analysis showed that the expression of TNNT1 mRNA in LUAD and LUSC cancer tissues was higher than that in adjacent normal tissues(P < 0.001),and the overall survival time of the group with high expression of TNNT1 mRNA in LUAD was shorter than that of the group with low expression of TNNT1 mRNA(P < 0.001).The prognosis is poor;Immunohistochemical results showed that the expression of TNNT1 was mainly high in advanced NSCLC tissues,while it was mainly low in control normal lung tissues.Correlation analysis combined with clinicopathological data showed that the expression of TNNT1 was correlated with lymph node metastasis(P=0.018)and TNM stage(P < 0.001).It was not correlated with age(P=0.194),sex(P=0.802),pathological type(P=0.756),degree of tumor differentiation(P=1.000),and tumor size(P=0.990).Kaplan-Meier survival curve analysis indicated that patients with high expression of TNNT1 had poor prognosis,and their overall survival and progression-free survival were shorter than those with low expression of TNNT1.Univariate regression analysis showed that the expression level of TNNT1(P < 0.001)and TNM stage(P=0.001)were important factors affecting the overall survival of patients with advanced NSCLC.Multivariate analysis showed that TNNT1 expression level(P<0.001)and TNM stage(P<0.001)were independent risk factors for predicting overall survival in patients with advanced NSCLC.Univariate analysis showed that TNNT1 expression level(P=0.001)and TNM stage(P=0.007)were also important factors affecting progression-free survival in patients with advanced NSCLC.Multivariate analysis showed that TNNT1 expression level(P < 0.001)and TNM stage(P < 0.001)were independent risk factors for progression-free survival in patients with advanced NSCLC.2.The expression level of TNNT1 in serum of NSCLC patients was(67.58±12.29)pg/m L,higher than that of healthy people(10.53 ± 3.15)pg/m L(P < 0.001).The expression level of serum TNNT1 was correlated with clinical stage and lymph node metastasis(P < 0.001),but not with gender,age and pathological type parameters(P >0.05).ROC curve analysis showed that the diagnostic efficiency of TNNT1,CEA and CYFRA21-1 combined detection was higher than that of TNNT1,CEA and CYFRA21-1 alone.Conclusion:1.TNNT1 is highly expressed in advanced NSCLC tissues,which is correlated with clinical stage and lymph node metastasis,and is an independent predictor of overall survival and progression-free survival.2.The expression level of TNNT1 is increased in the serum of patients with NSCLC,and the expression level of TNNT1 is correlated with clinical stage and lymph node metastasis.The detection of TNNT1 combined with CEA and CYFRA21-1 has better diagnostic efficacy for NSCLC.
