Objective: To observe the hemodynamic and resuscitation effects of ultrasound-guided iliac fascial space block(FICB)combined with remiazolam at different time points during hip fracture surgery in elderly patients.Methods: Sixty elderly patients who underwent hip surgery at Hefei hospital of Anhui Medical University between November 2021 and December 2022 were selected,and the patients were divided into a remimazolam group(Group R)and a propofol group(group B)by a random number table method,with 30 patients in each group.The two groups were induced using the same anesthetic drugs,and intraoperative maintenance was performed with pump rimazolam in Group R and with pump propofol in group B.SBP,DBP,HR,and BIS(bispectral index)were recorded for patients before anesthesia(T0),after induction of anesthesia(T1),at the beginning of surgery(T2),30 min after the beginning of surgery(T3),and at the end of surgery(T4)period;Time to anesthesia,procedure,extubation,and discharge from PACU were recorded;Adverse events such as nausea and vomiting,delayed awakening,dizziness and somnolence were recorded.Results:1.Comparison of general data: there was no statistically significant difference between the two groups in gender,age,ASA grade,BMI and anesthesia duration(P>0.05);2.Compared with T0,MAP and HR at T1,T2,T3 and T4 were significantly lower in both groups(P<0.05).There was no significant difference in MAP and HR between the three groups at T0,T1 and T2(P>0.05).At T3 and T4,MAP and HR in the control group were significantly lower than those in the experimental group(P<0.05).3.The extubation time and PACU time of patients in R group were significantly shorter than those in control group(P<0.05).4.Comparison of the incidence of adverse events between the two groups: there was no significant difference in the incidence of postoperative adverse reactions between the two groups(P>0.05),but the incidence of adverse reactions in group R was significantly lower than that in group P.Conclusion: Remimazolam had less haemodynamic effect on older hip surgery patients with general anesthesia combined with ultrasound-guided fascia iliaca block,with faster postoperative awakening,shorter extubation time,and fewer overall rates of adverse effects.
