| Objective:To investigate the distribution,changes of bacterial flora,drug resistance and the change of drug resistance trend of hospitalized patients with acute exacerbation of chronic obstructive pulmonary disease(AECOPD)in a tertiary hospital in Urumqi,Xinjiang Uygur Autonomous Region from 2013 to 2021.Methods:A retrospective analysis was performed on patients with AECOPD diagnosed by sputum culture in a hospital in Xinjiang from 2013 to 2021.Results:In the past nine years,the overall trend of bacterial infection in our hospital was decreased,while the trend of fungal infection was increased.The main infection strains were Gram-negative bacteria(G~-),followed by fungi,and Gram-positive bacteria(G~+)infection was the least.The top eight most common strains were Candida albicans(cal),Klebsiella pneumoniae(kpn),Pseudomonas aeruginosa(pae),Acinetobacter baumannii(aba),Escherichia coli(eco),Candida tropicalis(ctr),Stenotrophomonas maltophilia(pma)and Enterobacter cloacae(ecl),accounting for 22.23%,22.23%and 22.23%,respectively.21.49%,15.26%,10.04%,9.64%,3.82%,2.61%,2.21%.Chi-square test(χ~2)showed that age,smoking,underlying diseases,respiratory comorbidities and types of respiratory failure had an impact on the distribution of strains(P<0.0 5).BMI has an impact on respiratory failure in AECOPD patients.Among them,aba,eco and pae had high resistance probability to many antibiotics,and even completely resistant to some antibiotics.The overall resistance probability of kpn to antibiotics was significantly lower than that of the first three bacteria,with a high resistance probability to ampicillin and a resistance probability of less than 20.00%to the rest antibiotics.In recent years,fungi have also emerged drug resistance,but the drug resistance rate is not high at present,and the highest is not more than 10.00%.Gram-positive bacteria also have high resistance to some antibiotics or even complete resistance.In recent years,the proportion of fungi began to increase,the proportion of bacteria decreased,and the overall proportion of G+bacteria did not change significantly.Multiple factors affect the distribution of AECOPD strains.Conclusion:In recent years,the proportion of fungi began to increase,the proportion of bacteria decreased,and the overall proportion of G+bacteria did not change significantly.Multiple factors affect the distribution of AECOPD strains.The drug resistance of bacteria was generally on the rise,and fungi also began to show drug resistance in recent years,but the drug resistance rate was not high at present.The overall drug resistance rate of G+bacteria was also on the rise. |
