| Objective: to observe the characteristics of dermatoscope and wood lamp of different syndromes of vitiligo in Uyghur medicine,so as to provide a basis for more objective and accurate judgment of abnormal body fluid classification in Uyghur medicine.Methods: this study collected 300 patients with vitiligo diagnosed in the Department of Dermatology of Xinjiang Uyghur Medical Hospital from February 2020 to2023,which were astringent mucinous type,salty mucinous type,sour mucinous type,tasteless mucinous type,plaster-like mucinous type and sweet mucinous type.The dermatoscope and wood lamp were used to observe and record the background color,vascular morphology,boundary condition,abnormal hair,hair follicle,pigmentation of the skin,skin surface and skin surface texture features of abnormal mucinous type in Uyghur medicine.Results: 1.In this study,there were more females than males,with the largest number of patients with new history of vitiligo and sporadic vitiligo,mainly concentrated in the face,neck,upper limbs,abdomen and lower limbs(P<0.05).2.The recurrence history,clinical classification and location of vitiligo patients with different syndrome types were statistically significant(P<0.05).3.Observation by dermatoscope:1)The background color of patients with astringent and salty mucinous vitiligo was milky white,and the difference was statistically significant(P<0.05).2)The blood vessel morphology of patients with astringent mucinous vitiligo was not visible,and the difference was statistically significant(P < 0.05).3)The boundary of patients with astringent mucinous vitiligo was blurred and the difference was statistically significant(P<0.05);4)The skin pigmentation deepens,the leukoplakia and surrounding skin have light red spots,and the pericystic pigment remains.The ratio of white hair in astringent mucinous type vitiligo patients is higher than that of the other five abnormal body fluids type Vitiligo;Peripheral punctate and irregular hypopigmentation spots,scattered punctate leukoplakia of normal skin around leukoplakia,patchy pigment residues can be seen in the leukoplakia area,skin hypopigmentation spots in the follicles are salty mucinous type.The rate of occurrence in mucinous type vitiligo is higher than that of the other five abnormal mucinous types of vitiligo;The skin texture around the leukoplakia is clearly visible,and the rate of occurrence in astringent and salty mucinous type vitiligo is higher than that of the other four abnormal mucinous types of vitiligo;The skin texture in the leukoplakia area is clear,with light red,and the ratio of the tasteless and sour mucinous type of vitiligo is higher than that of the other four types of abnormal mucinous type of vitiligo,and the difference is statistically significant(P<0.05).4)Observation with Wood’s lamp: The color of astringent mucinous vitiligo was bluish-white or off-white under wood lamp,and the difference was statistically significant(P<0.05).Conclusion: 1.Astringent mucinous type vitiligo has the most relapses within 1 to 3months,and the clinical classification is most sporadic,and most of the onset sites are upper limbs.2.The manifestations of abnormal mucinous type vitiligo under dermoscopy are as follows: 1)Astringent mucinous vitiligo:(1)milky white background;(2)blood vessels are hard to see;(3)the boundary is fuzzy;(4)white hair;(5)residual pigment around hair follicle;(6)percutaneous pigment deepens;(7)The skin texture around the white spot is clearly visible;(8)white spots and the surrounding skin with light red spots.2)Salty mucinous vitiligo:(1)milky white background;(2)dotted leukoplakia around the normal skin;(3)The skin texture around the white spot is clearly visible;(4)peripheral punctate and irregular hypopigmentation spots;(5)patchy pigment residue can be seen in the white spot area;(6)hair follicle skin pigment loss spot;3)Sour mucinous vitiligo:(1)yellow-white background;(2)white spot area skin texture clear,with light red;4)tasteless mucinous vitiligo: white spot area skin texture clear,with light red.The above characteristic dermoscopic images can be used as the diagnostic basis for the differential diagnosis and classification of vitiligo with abnormal mucinous vitiligo.3.Astringent mucinous vitiligo appears bluish-white or off-white under wood lamp. |
