Comparative Study Of Longitudinal Incision And Sail Incisure Incision In The Treatment Of Carpal Tunnel Syndrome

Objective:By comparing the longitudinal incision with the sail incisure incision in the treatment of Carpal tunnel syndrome,it provides a reference for clinical treatment.Methods:From September 2020 to September 2022,52 patients(67 wrists)with moderate and severe Carpal tunnel syndrome in the outpatient,emergency and inpatient departments of the First Affiliated Hospital of Heilongjiang University of Chinese Medicine were randomly divided into longitudinal incision group(group A,n=26,32 wrists)and windsurfing incision group(group B,n=26,35 wrists).The average postoperative follow-up was more than half a year.The intraoperative conditions(incision length,intraoperative bleeding,operation time)and hospital stay,clinical efficacy(Kelly grade excellent rate,GSS,BCTQ),neuroelectrophysiological outcome(SCV,SNAP,DML,CMAP),and postoperative complications(infection,scar pain,postoperative hematoma,nerve injury)of the two groups were compared.Results:There was no statistically significant difference in the preoperative clinical data between the two groups(P>0.5),which was comparable.The incision length,intraoperative bleeding,operation time and hospital stay in group B were smaller than those in group A,and the difference was statistically significant(P<0.001);The excellent and good rate was 87.5% in Group A and 97.14% in Group B,with no significant difference(P>0.05);The GSS(pain,numbness,peripheral nerve sensation,muscle weakness,night awakening,total score)of Group B was better than that of Group A,and the difference was statistically significant(all P<0.05);The BCTQ(symptom score and function score)of patients in group B was better than that in group A,and the difference was statistically significant(P<0.05);There was no significant difference between the two groups in neuroelectrophysiological indexes(SCV,SNAP,DML,CMAP)(all P>0.05);The total postoperative complication rate of group A was37.5%,and that of group B was 8.75%,the difference was statistically significant(P<0.05).Conclusion:Compared with the longitudinal incision,the windsurfing incision has shorter incision length,less intraoperative bleeding,shorter operation time and hospital stay,better GSS score and BCTQ score,and fewer complications,which is worthy of clinical application and promotion.
