Clinical Observation Of Yi Kidney Formula With Cangfu Daotan Decoction In Treating PCOS With Kidney Deficiency And Phlegm-Dampness

Objective:In this study,we discussed the clinical efficacy of Yi Kidney Formula with Cangfu Daotan Decoction in the treatment of PCOS patients with kidney deficiency and phlegm-dampness by observing the overall efficiency,the changes in the traditional Chinese medicine（TCM）syndrome score,physical signs and some related indexes of auxiliary examination before and after treatment,providing objective basis for clinical application.Methods:In this study,40 patients with PCOS who met the criteria of kidney deficiency and phlegm-dampness were treated with Yi Kidney Formula with Cangfu Daotan Decoction for 3 consecutive menstrual cycles.SPSS26.0 was used for statistical analysis by observing the total effective rate after treatment,changes in TCM syndrome score,physical signs,serum reproduction-related hormone levels,glycolipid metabolic indexes,ovarian morphological indexes and liver function indexes before and after treatment.Results:A total of 40 patients were clinically observed,among which 3 cases fell off.37 patients completed the course of treatment.（1）Overall efficacy:4 cases were cured,10 cases showed efficacy,18 cases were effective,5 cases were ineffective,with an overall effective rate of86.49%.（2）Curative effect of TCM syndrome:after treatment,the score of menstrual cycle,menstrual volume,soreness and weakness of the waist and knees,chest and abdominal fullness,physical obesity,dizziness and tinnitus,menstrual color,menstrual quality and total score were both significantly reduced,and the difference was statistically significant（P<0.01）.Although the symptoms of mental fatigue and energy were improved,the difference was not statistically significant.（3）Changes in physical signs:BMI,WHR,acne score and acanthosis nigricans score were significantly decreased after treatment,with statistical significance（P<0.01）;hirsutism signs were improved,but there was no statistical difference.（4）Serum reproduction-related hormone indexes:serum LH,LH/FSH and T were significantly decreased,E₂and SHBG were increased after treatment,all of which were statistically different（P<0.01）;while serum FSH,PRL,P,4-AD,DHEAS and AMH were not significantly changed,with no statistical difference.（5）Glycolipid metabolism indexes:after treatment,serum FPG,FINS,TG,LDL-C and HOMA-IR were significantly decreased,with statistical difference（P<0.05）;the changes of serum TC and HDL-C were not statistically different.（6）Morphological index of ovary:the number of follicles in sinus decreased significantly after treatment,and the difference was statistically significant（P<0.01）.There was no significant change in ovarian volume and no statistical difference.（7）Liver function indexes:there was no statistical difference in ALT and AST before and after treatment.Conclusions:The curative efficacy of Yi Kidney Formula combined with Cangfu Daotan Decoction in the treatment of PCOS patients with kidney deficiency and phlegm-dampness is relatively significant,which can improve clinical symptoms such as menstrual cycle or menstrual volume and physical signs such as obesity,acne and acanthosis nigricans.It can effectively increase the levels of serum E₂and SHBG,reduce the levels of serum LH,T,FPG,FINS,TG,LDL-C and IR,improve the disorder of glucose and lipid metabolism,and reduce the number of sinus follicles.No obvious adverse reactions were found in the clinical application of this protocol,which is safe and worthy of further promotion and application.
