| Objective: To analyze the factors affecting the survival cycle based on the clinical data of 73 gallbladder cancer patients,and to predict whether lymph node metastasis and distant metastasis occur in gallbladder cancer patients by the expression level of serum tumor markers.Methods :The clinical case data of 81 gallbladder cancer patients with clear diagnosis admitted to our hospital from January 2017 to December 2021 were collected,and the patients who received treatment were followed up by telephone,and the follow-up content was patients’ survival status,and the end event of follow-up was patients’ death or the deadline of follow-up was December 31,2021.73 patients with complete follow-up data were finally selected as the study subjects.Kaplan-Meier univariate analysis of factors affecting survival,Cox regression model analysis of independent risk factors affecting prognosis,and ROC curve plotting to predict the clinical value of serum tumor markers on the occurrence of lymph node metastasis and distant metastasis in patients with gallbladder cancer.Results :In this paper,the clinical data of 73 gallbladder cancer patients were analyzed,including 34 males and 39 females,aged 41-88 years,with a median age of 73.0±19.5 years,of which 36 cases(49.3%)were under 73 years old and 37 cases(50.7%)were over 73 years old.Univariate analysis showed that tumor markers(CEA,CA199),lymph node metastasis,distant metastasis,TNM stage,surgical method,and Child classification were factors affecting the prognosis of patients with gallbladder cancer(P< 0.05),while gender,age,presence of cholecystitis or stones,and the presence of concomitant underlying diseases had no effect on the prognostic survival cycle of patients.Further multifactorial analysis showed that lymph node metastasis,distant metastasis and surgical modality remained independent factors affecting prognostic survival of patients with gallbladder cancer(P< 0.005).Evaluation of serum CEA,CA199 and CA125 expression levels in GBC patients according to ROC curves for predicting the occurrence of lymph node and distant metastasis showed that serum CA125 alone versus CEA combined with CA125 had the highest sensitivity for the occurrence of lymph node metastasis(93.5%),while CA199 combined with CA125 had the highest specificity for lymph node metastasis(85.2 The diagnostic value of the combination of CEA,CA199 and CA125 was significantly higher than that of CEA,CA199 and CA125 alone for the development of distant metastases.93.3%).Conclusion :The prognostic survival cycle of gallbladder cancer patients is influenced by many factors,and this paper concluded that lymph node metastasis,distant metastasis,and surgical method are independent risk factors affecting the prognostic survival of gallbladder cancer patients,and screening of patients with combined high-risk factors should be strengthened,and the disease progression should be judged based on the expression level of tumor markers for early and effective clinical intervention. |
