| Objective:This study collected the data of patients with osteomalacia to investigate the etiology and clinical characteristics of osteomalacia.To improve the understanding of osteomalacia.Methods:Collect the clinical,biochemical and imaging data of 34 patients with osteomalacia from Xinjiang Uygur Autonomous Region People’s Hospital from 2013to 2021,the clinical characteristics and etiological composition of osteomalacia patients were analyzed and followed up.Patients with clinically diagnosed osteomalacia were selected as case group,and non-osteomalacia patients in hospital at the same time matched1:4 according to age and sex were selected as control group,the clinical characteristics and biochemical indexes were compared between the two groups.Results:(1)In 21patients,osteomalacia was caused by vitamin D deficiency(76.19%),and osteomalacia was caused by hypophosphatemia(23.81%).(2)The average age of the patients with low phosphorus osteomalacia was 53.60?12.37 years old,the average body mass index was20.02?11.24 kg/m~2,the average serum calcium was 2.18?0.24 mmol/l,the average serum phosphorus was 0.36?0.10 mmol/L,the interquartile range of 25-hydroxyvitamin D was31.75(19.26,46.98)ng/ml,the interquartile range of alkaline phosphatase was253(170.47,397.83)U/L,and the mean Parathyroid hormone was 144.70?130.23 pg/ml,the average 24-hour urine calcium was 6.22?2.91mmol/l,the average 24-hour urine phosphorus was 25.61?19.75 mmol/L.The mean age of patients with osteomalacia due to vitamin D deficiency was 36.10?10.86 years old,the mean body mass index was20.65?3.14 kg/m~2,the mean serum calcium was 2.05?0.25 mmol/l,the mean serum phosphorus was 0.82?0.22 mmol/L,the mean 25-hydroxyvitamin D was 7?4.63ng/ml,the alkaline phosphatase interquartile range was 338(206.22,429.50)U/L,the mean Parathyroid hormone was 408.50?331.62pg/ml,and the mean 24-hour urinary calcium was 2.92?1.82mmol/L,the average 24 hours urine phosphorus was12.40?11.38mmol/L.(3)Comparedwithbeforetreatment,serum phosphorus(0.75?0.20VS0.98?0.27,P<0.001),25-hydroxyvitamin d(10.73?5.99VS24.45?14.02,P=0.010),parathyroidhormone(453.10?392.85VS169.26?149.29,P=0.013),procollagen type I N Propeptide(309?191.45VS127.21(76.55,220.47),P=0.036),C-terminal cross-linking telopeptide of type I collagen(2.23?1.35VS1.13?0.71,P=0.031),incidence of bone pain symptoms(100%VS10%,P=0.011)andwalkingrestriction scorewere(3.5(3,4)VS2.5(2,3),P=0.005)improved in patients with osteomalacia due to vitamin D deficiency after treatment.Conclusion:The main causes of osteomalacia in this study included low phosphorus osteomalacia and vitamin D deficiency osteomalacia.The main clinical manifestations were bone pain and muscle weakness,biochemical characteristics include hypophosphatemia,hypocalcemia,vitamin D deficiency,elevated Alkaline phosphatase and Parathyroid hormone.The clinical characteristics of osteomalacia vary with different etiology.After active treatment,the clinical and biochemical manifestations of patients with osteomalacia was caused by vitamin D deficiency were relieved. |
