Effects Of Resting Heart Rate,Obesity And Their Interaction On Type 2 Diabetes

Abstract/Summary:
Objectives:To analyze the internal relations of resting heart rate and obesity and the risk of type 2 diabetes(T2DM),and to investigate whether there is an additive interaction between high resting heart rate and obesity on T2DM.Methods:This subject was a cross-sectional study which using stratified cluster random sampling method.The subjects were 11956 permanent residents over 35 years old who involved in the Cardiovascular Health Study in Northeast China from 2012 to2013.After screening,10059 respondents were finally included.SPSS 26.0 statistical software was used for data analysis,both of which were bilateral tests,and P<0.05 was statistically significant.χ~2test and t test(or rank sum test)were used to evaluate the distribution of categorical variables and continuous variables in T2DM patients and control group respectively.The characteristics of T2DM and demography were described by the mean and its standard deviation or composition ratio.Using unconditional logistic regression analysis,a multifactor adjusted model was established to analyze two different definitions of obesity by WC and BMI,and the risk of T2DM in different resting heart rate groups.We used logistic regression to analyze whether the joint effect of resting heart rate and obesity on the risk of T2DM was higher than that of the two alone.Appling the Excel which was prepared by Anderson et al.,the Synergy index,attributable proportion due to interaction and the relative excess risk due to interaction were counted,then analyzed whether there was additive interaction between resting heart rate and obesity in the light of the index results.Results:1.In the study participants,there were 1049 patients in T2DM group,with a prevalence rate of 10.43%.The basic point of two different groups which distinguished by high and low blood sugar were compared.The differences between the two groups were dissimilarity significant except for drinking history,and nationality family history of coronary heart disease.2.By adjusting variates which were confounding factors,the risk of T2DM in the high resting heart rate group was 2.272 times higher than that in the regular resting heart rate group(OR:2.272,95%CI:1.983,2.604);The risk of T2DM in obese people defined by BMI is 1.817 times higher than that in in overweight and below(OR:1.817,95%CI:1.561-2.116),and the risk of T2DM in abdominal obese people defined by WC is 2.018times higher than that in normal people(OR:2.081,95%CI:1.792,2.416),the difference is statistically significant.3.The combination of resting heart rate and obesity defined by BMI and WC and the risk of T2DM were analyzed by logistic regression.It was found that when high resting heart rate and obesity under different definitions exist alone,they are both associated with the risk of T2DM,and the risk of T2DM of subjects with both exists is higher than that of high resting heart rate or obesity alone.4.On the basis of the outcomes of the additive interaction,there is an additive interaction between high resting heart rate and obesity defined by BMI and WC on the occurrence of T2DM.By adjusting variates which were confounding factors,the risk of T2DM is owe to the risk of interaction between high RHR and obesity defined by the two definitions,which is 1.045 and 1.916 respectively;In all T2DM cases,the interaction between high resting heart rate and obesity under two definitions accounted for 24.9%and 37.6%respectively;The effects of high resting heart rate and obesity under the two definitions are 1.484 times and 1.877 times higher than those of high resting heart rate and obesity when they exist independently.Conclusion:1.High resting heart rate(RHR>80 beats/min)is positively correlated with the risk of T2DM.2.Both general obesity defined by BMI and abdominal obesity defined by WC are positively correlated with the risk of T2DM.3.Subjects with high resting heart rate and obesity under both definitions have a higher risk of T2DM than subjects with high resting heart rate and obesity alone.4.There is an additive interaction between high resting heart rate and obesity under two definitions on the risk of T2DM.
