Objective: The study aimed to investigate the association between pulmonary function and MCI among adults in rural areas of Liaoning,China,in order to provide a scientific basis for reducing the prevalence of MCI,delaying the progression of MCI to dementia,and formulating relevant intervention measures.Methods: A cross-sectional study was used to collect information about 4689 subjects(≥35 years old)who met inclusion and exclusion criteria through questionnaires,physical examinations,and pulmonary function tests.The Chinese version of the Montreal Cognitive Assessment Basic(Mo CA-BC)to assess cognitive impairment.The diagnostic criteria for MCI are based on those developed by the National Institute on Aging and the Alzheimer’s Association;Pulmonary function was measured using a calibrated Chest HI-101,which recorded forced vital capacity(FVC),forced expiratory volume in the first second(FEV1),and peak expiratory flow(PEF).The best values of FVC,FEV1 and PEF were standardized into z scores by using the means and standard deviations computed from the entire study,and we created a composite pulmonary function score by averaging the z-scores for FVC,FEV1,and PEF and then categorized them into four categories for the study.Binary logistic regression models were used to estimate the odds ratios(ORs)and 95% confidence intervals(CIs)for composite pulmonary function and MCI.Moreover,we used restricted cubic splines(RCS)based on logistic regression models to visualize the relationship between pulmonary function and MCI by adjusting the same confounders.On this basis,we investigated the correlation between pulmonary function and MCI risk in different subgroups.Sensitivity analysis was also performed to verify the stability of the results.Results: In Total,the mean age(mean ± SD)of the subjects in this study was57.5±9.8 years old.1939 were females,accounting for 65.8% of the total study population.The prevalence of MCI in the participants decreased with increasing pulmonary function,being 63.9%(471/737),50.5%(372/736),43.8%(323/738)and43.6%(321/736)in the lowest to highest quartiles of pulmonary function,respectively.After adjustment for confounding factors,participants in the fourth quartile had a significantly reduced risk of developing MCI compared with those in the first quartile of lung function Z-score(OR:0.59,95%CI :0.44-0.79);The results of trend test were also statistically different(P trend <0.001).The limited cubic spline showed an approximate L-shaped nonlinear correlation between pulmonary function and MCI(P nonlinearity =0.032).we only found a relationship between lower pulmonary function and a higher risk of MCI in the females.Conclusions: Pulmonary function was inversely associated with the risk of MCI.The rural population with low pulmonary function should pay more attention to their own cognitive function as soon as possible and carry out early cognitive assessment.
