Objective: The purpose of this experiment is to explore the effect of preoperative anxiety on the quality of recovery of patients who plan to undergoing gynecological hysterectomy. Methods: From October 1,2021 to November 30,2022,patients scheduled for elective gynecological hysterectomy were enrolled in the study. The State Anxiety scale(S-AI)was the measure for preoperative anxiety one day before the operation.According to this score, they were divided into two groups: Group H(moderate and severe anxiety group,S-AI score ≥ 40 points)and Group L(mild anxiety group,S-AI score<40 points). The primary endpoint was the postoperative recovery quality(Qo R)which was assessed using Quality of Recovery Scale40(Qo R-40)on the first day after operation.Other endpoints include the time of the first exhaust and defecation after operation, the time of the first intake of clear liquid,semi-liquid food and liquid food, the time of hospitalization, the time of anesthesia, and the time of the post-anesthesia care unit(PACU)after anesthesia. This study has been approved by the Ethics Committee of Shengjing Hospital affiliated to China Medical University, with the ethics number of 2021PS683 K,and registered in the China Clinical Trial Registration Center, with the registration number of Chi CTR220065837.Results: They were divided into two groups: 138 people in Group H and 227 people in Group L,a total of 365 people were included in the statistical analysis. The Qo R-40 score of patients with mild preoperative anxiety after hysterectomy was higher than that of patients with moderate and severe anxiety one day after surgery(P<0.001).The Qo R-40 scores of the patients planning to undergoing gynecological hysterectomy were negatively related to preoperative S-AI score(correlation coefficient TAU=-0.499,P<0.001).Multiple linear regression showed that S-AI score was an independent related factor for postoperative recovery of Qo R-40 score(β=-0.73,P<0.001).Conclusion: For patients undergoing elective gynecological hysterectomy, preoperative anxiety state is an independent related factor of postoperative recovery quality. The higher the preoperative anxiety, the worse the quality of postoperative recovery.
