Retrospective Analyses On 227 Western-Chinese Combination Treatments For Chronic Actinic Dermatitis

Oobjective:To summarize the clinical characteristics,treatment rules of combined Chinese and Western medicine and clinical efficacy of CAD through the collation and analysis of clinical data of patients hospitalized with chronic actinic dermatitis(CAD)in the Department of Dermatology,The First Affiliated Hospital of Yunnan University of Chinese Medicine,and to provide reference for the dialectical treatment of chronic actinic dermatitis with combined Chinese and Western medicine.Method:Data were obtained from patients with chronic actinic dermatitis admitted to the Department of Dermatology,Yunnan Provincial Hospital of Traditional Chinese Medicine from January 2017 to December 2021.A retrospective study of medical records was used: general information of patients(such as:gender,age,duration of disease,occupation,etc.),clinical manifestations(such as:symptoms,lesion morphology,lesion distribution),syndrome differentiation of traditional Chinese medicine,laboratory tests and Chinese and Western medicine diagnosis,treatment methods,and regression were collected and collated to establish a database of patient data,and statistical analysis of this date using SPSS26.0.Results:1.General information: 227 medical records were collected,203 cases were male and 24 cases were female,with a male to female ratio of 1:0.12;the shortest duration of illness was 4 months and the longest was 40 years,with 74.9% of patients having a duration of illness of more than 1.5 years;occupation was more common among farmers,retirees,workers and other frequent outdoor activities.The shortest hospitalization days were 5 days and the longest 2 weeks,the average hospitalization days were 7.58±1.39 days.The number of cases is higher in spring and summer,but the overall difference is not significant.2.Disease-related history: 189 indicated that sun exposure led to recurrence or aggravation of the disease;57 patients had a history of drug allergy,with drugs such as sulfonamide,penicillin,cephalosporin and other antibiotics being the most common.3.Laboratory tests: routine blood abnormalities were mainly increased eosinophils and leukocytes;34 patients had increased sedimentation;52 patients had increased c-reactive protein.Food-specific IgG was prevalent in 1-2,most commonly:mushrooms,eggs,beef and crab.4.Characteristics of skin lesions and TCM syndrome types: all patients’ skin lesions involved the head,face and neck,and the longer course of the disease was easy to involve the trunk;Less ear involvement.Erythema,papules and pruritus of different degrees were found in 227 patients,and most of them had invasive erythema in the acute stage.With erosion,exudation less.There were 120 cases(52.9%)with dampness-heat toxic syndrome.There was no difference in age,course of disease.5.Treatment: 100% of cases were treated with a combination of Chinese and Western medicine.9 cases were cured,212 cases were improved,and 6 cases were not cured,with a total effective rate of 97.36%.Conclusion:1.Chronic actinic dermatitis tends to occur in middle-aged and elderly men,with a long duration of illness that is difficult to cure;the incidence is higher in spring and summer;the average hospital stay is about 1 week.2.The lesions are more common on the head,face,neck and extremities,but rarely on the ears.3.The patient’s symptoms are mainly damp-heat and toxicity,and are reduced by adding and subtracting gentian soup;blood-heat and wind-dryness are second,and are reduced by adding and subtracting scutellaria soup;overall,most of the symptoms are solid,but fewer are mixed with deficiency.4.The main treatment for chronic actinic dermatitis at our hospital is a combination of TCM dialectic therapy with immunomodulators,antihistamines and drugs to reduce capillary permeability,supplemented by topical hormone ointment and TCM external treatment.5.The combination of Chinese and Western medicine is effective in treating chronic actinic dermatitis.
