Purpose of the studyThe purpose of this study was to analyze the correlation between the expression of villin in patients with pancreatic cancer and clinical counting data,to observe the difference of disease-free survival among patients with pancreatic cancer with different levels of villin expression,and to evaluate the predictive value of villin expression level in survival and prognosis of patients with pancreatic cancer.To explore whether early screening and diagnosis of potential pancreatic cancer patients can be made by observing the expression of villin,and whether the relevant clinical indicators can be regulated to improve the prognosis of pancreatic cancer patients.Research methodsA total of 100 patients with ductal adenocarcinoma were treated in the Department of Hepatobiliary Pancreatic and splenic surgery of the affiliated Hospital of Inner Mongolia Medical University from 2018.01 to 2021.12.56 patients were selected according to the inclusion and exclusion criteria.The surgical specimens of 56 patients were divided into two groups according to pancreatic cancer tissues and paracancerous tissues.The expression of villin in cancer and paracancerous tissues was detected by immunohistochemistry(Smurp method),and the expression of villin in the two groups was evaluated.According to the expression of villin in cancer tissues,villin negative expression and weak positive expression were divided into A group,moderate positive expression and strong positive expression were divided into B group.SPSS 25.0 software was used to analyze the expression intensity of villin in patients with pancreatic cancer and sex,smoking,drinking,diabetes,hypertension,age,hospitalization days,DFS,lymph node metastasis,jaundice,tumor invasion,tumor differentiation,tumor pathological stage,tumor diameter,Alanine Transaminase(ALT),Aspartate Aminotransferase(AST),glutamic oxaloacetic transaminase mitochondrial type,Total Bile Acid(TBA),Gamma-Glutamyl transferase(γ-GGT),Glutamate Dehydrogenase(GLDH),Alkaline Phosphatase(ALP),Total Bilirubin(TBIL),Direct Bilirubin(DBIL),Indirect Bilirubin(IBIL),Total Protein(TP),Albumin(ALB),Prealbumin(PA),Glucose(GLU),α-Fetoprotein(AFP),Carcinoembryonic Antigen(CEA),Ferritin,CA-125 and CA-199 were analyzed.Kaplan-Meier method was used to evaluate the survival analysis of patients in group An and group B,and the difference of DFS between the two groups was evaluated and the survival curve was drawn.The expression intensity of villin,clinical count and quantitative indexes were included to construct a single-factor and multi-factor Cox proportional risk model to analyze the independent prognostic factors of DFS in patients with pancreatic cancer.Research resultsThe comparison of villin expression in pancreatic cancer tissues and adjacent tissues showed that among the 56 cases of pancreatic cancer,18 cases were negative(32.1%),8 cases were weakly positive(14.3%),14 cases were positive(25.0%),and 16 cases were strongly positive(28.6%).Among the 56 cases of paracancerous tissues,52 cases were negative(92.9%),4 cases were weakly positive(7.1%),and 0 cases were positive and strongly positive(0%).Using multiple rate comparison X2 test,statistical analysis showed that the expression of villin between pancreatic cancer and paracancerous tissues was statistically significant(P < 0.001).The differences of clinical count and metrological data between group An and group B were analyzed by X2 test and rank sum test.The results showed that there were significant differences in hospital stay,ALT,DBIL,AST,glutamic oxaloacetic transaminase mitochondrial type,IBIL,γ-GGT,GLDH,ALP,TBA,TBIL,ferritin and CA-125 between the two groups (P<0.05).There was no significant difference with PA,TP,ALB,GLU,AFP,CEA,CA-199,tumor diameter and age distribution of patients(P>0.05).The survival analysis of 56 patients in group An and group B by Kaplan-Meier method showed that the DFS of group A was 27(95%CI 24.509-29.491)months,and the DFS of group B was 17(95%CI 14.700-19.300)months.According to the intensity of villin expression,degree of jaundice,surrounding tissue invasion,tumor differentiation,tumor pathological stage,ALT,DBIL,AST,glutamic oxaloacetic transaminase mitochondrial type,γ-GGT,GLDH,ALP,TBIL,IBIL,ferritin,CA125,CA199 and other indexes,multivariate Cox regression was established: jaundice degree(HR=0.513,95%CI 0.266-0.987,P=0.045),tumor invasion to surrounding tissue (HR=1.543,95%CI 1.097-2.171,P=0.013),tumor differentiation(HR=49.097,95%CI13.444-179.301,P=0.001),pathological stage(HR=2.819,95%CI 1.887-4.212,P=0.001)and glutamic-pyruvic transaminase(HR=1.011,95%CI 1.006-1.017,P=0.001)were independent prognostic factors for DFS in patients with pancreatic cancer.However,the expression of villin and other indexes were not independent prognostic factors of DFS in patients with pancreatic cancer(P>0.05).Experimental conclusion1.The expression of villin in pancreatic cancer tissues was higher than that in paracancerous tissues,and the difference between them was statistically significant.2.There were significant differences in jaundice,tumor differentiation,hospital stay,DFS,ALT,AST,glutamic oxaloacetic transaminase mitochondrial type,γ-GGT,GLDH,ALP,TBA,TBIL,IBIL,DBIL,ferritin and CA-125 between group An and group B.3.The expression of villin is the reason for the difference in DFS and survival rate between the two groups.The overall survival rate of group An is better than that of group B.4.In univariate COX regression analysis,the expression of villin had a significant effect on DFS in patients with pancreatic cancer.After the collinearity was eliminated in multivariate COX regression,it was found that villin was not an independent prognostic factor for DFS in patients with pancreatic cancer.
