| Objective1.Investigate the current situation of professional self-concept,retention intention and job burnout of emergency nurses in primary medical institutions,and analyze the influencing factors.2.On the basis of the investigation and research,the intervention plan was constructed,the expert assistance was carried out under the medical community model,and the effect of intervention was observed,so as to provide a basis for the capacity building of primary emergency nursing service and the stability of nursing team.Methods1.Investigation on professional self-concept,retention intention,job burnout of primary emergency nurses and its influencing factors: 203 emergency nurses engaged in emergency care in 10 primary hospitals in Suzhou,Jiangsu Province from September 2021 to November 2021 were selected as the research objects.General data questionnaire,professional self-concept scale,retention intention scale and job burnout scale were used to investigate the general demographic data,professional self-concept,retention intention and job burnout of primary emergency nurses.SPSS26.0 software was used to record and statistically analyze the data to verify the main factors affecting the professional self-concept of primary emergency nurses.P<0.05 indicated that the difference was statistically significant.2.Intervention study on professional self-concept,retention intention and job burnout of primary emergency nurses assisted by medical community experts: 45 emergency nurses from 3 primary member units of medical community affiliated to a tertiary hospital in Suzhou,Jiangsu Province from December 2021 to February 2023 were selected as research objects.Focus group interviews were conducted to clarify the contents of assistance,and the methods of assistance such as stagnation point sinking to the grassroots,sending technology to the countryside,and selecting and training upwards were explored under the medical community model.Assistance plans were formulated,comprehensive assistance programs were constructed,and one-year assistance intervention was carried out.The professional self-concept,retention intention and job burnout status of the study subjects were compared before and after the implementation of the help program.Data were recorded and statistically analyzed by SPSS 26.0 software to verify the effect of intervention.P<0.05 indicated that the difference was statistically significant.Results1.The survey showed that the professional self-concept score of primary emergency nurses(70.37±8.52);Retention intention score(21.29±4.04);The score of job burnout and emotional exhaustion dimension(24.11±7.62),the score of depersonalization dimension(11.28±5.96)and the score of reduced personal achievement dimension(25.79±7.71).Among 203 primary emergency nurses,77 had positive professional self-concept(37.93%),88 had high retention intention(43.35%),and 144 had severe job burnout(70.94%).Univariate analysis showed that there were statistically significant differences in the number of people who received new technology and theory training,humanistic nursing training,first-aid knowledge and skills training and nursing management training(P < 0.05).2.After the expert assistance,the professional self-concept score of primary emergency nurses in the medical community reached the positive state(82.27±2.23),and the number of active nurses was 30(66.67%),which was higher than before;The score of retention intention(24.53±0.63)reached the level of high retention intention,and the number of high retention intention was 28(62.22%),which increased.The score of emotional burnout dimension(21.78±1.51),the score of depersonalization dimension(7.80±0.49)and the score of reduced personal sense of achievement dimension(39.44±0.75)showed that job burnout was less than before.The difference before and after intervention was statistically significant(P < 0.05).Conclusions1.Primary emergency nurses lack the training of new technology and theory,nursing management,humanistic nursing and first-aid knowledge and skills,leading to the negative professional self-concept,weak retention intention and serious job burnout.2.Targeted expert assistance for primary emergency nurses under the medical community model can improve nurses’ professional self-concept and retention intention,and alleviate their job burnout.It is beneficial to stabilize the nursing team,increase the enthusiasm and satisfaction of nurses,and improve the ability of primary emergency nursing service. |
