| OBJECTIVE: To investigate the clinical effectiveness of adding or subtracting Bao Yuan Tang in treating cancer-related fatigue with evidence of spleen and kidney yang deficiency.METHODS: A total of 62 eligible patients were enrolled in this study and randomly divided into two groups: the control group(n=31)and the experimental group(n=31).The control group received standard western medical care,including symptomatic treatment for tumorrelated complications,post-treatment complications,pain management,and conventional rehydration and nutritional support for four weeks.The experimental group received Pao Yuan Tang plus reduction on top of the standard care for Piper’s fatigue for four weeks.The Piper Fatigue Scale,TCM symptom scores,Carlsbad scores,sleep quality scores,white blood cell counts,and neutrophil counts were collected before and after the intervention.The collected data were entered into a database,collated,and analyzed using appropriate statistical methods.RESULTS: 1.Comparison of Blood Counts Before and After Intervention: The white blood cell count and neutrophil count in the test group were found to be significantly higher(P<0.05)after the intervention compared to their levels before the intervention.Similarly,in the control group,although the leukocyte count and neutrophil count were elevated after the intervention,the difference was not statistically significant(P>0.05).On comparing the test group with the control group after the intervention,it was observed that the white blood cell count and neutrophil count in the test group were significantly higher than those in the control group,with a statistically significant difference(P<0.05).2.Changes in Piper Fatigue Survey Scale scores: Comparison of the pre-intervention Piper Fatigue Scale total scores,behavioral dimension scores,affective dimension scores,sensory dimension scores,and cognitive dimension scores showed that the differences between the pre-intervention scores of the test and control groups were not statistically significant(P>0.05)and were comparable.Comparison of the post-intervention Piper Fatigue Scale total scores,behavioral dimension scores,affective dimension scores,sensory dimension scores and cognitive dimension scores showed that the Piper Fatigue Scale total scores,behavioral dimension scores,affective dimension scores,sensory dimension scores and cognitive dimension scores improved and were statistically significant(P<0.05).3.Changes in the Kahl score: Following the intervention,there was a statistically significant improvement(P<0.05)in Kahl score in the test group compared to their pre-intervention scores.Conversely,in the control group,the post-intervention Kassel score increased compared to pre-intervention score,but the difference was not statistically significant(P>0.05).For the comparison of the data before and after the intervention,the results suggested that the difference between the two groups’ karst scores before the intervention was not statistically significant(P>0.05),and the difference between the two groups’ karst scores after the intervention was statistically significant(P<0.001).4.Changes in TCM symptom scores: there was a statistically significant difference in the efficiency rate between the test and control groups after the intervention(P<0.001).It is suggested that the TCM symptoms were improved after the intervention.5.Change of sleep condition: Following the intervention,there was a statistically significant improvement(P<0.05)in the rate of sleep improvement and efficiency rate in the test group compared to the control group,indicating an improvement in sleep quality after the intervention.CONCLUSIONS: The formula developed by Po-Yuan Tang,along with reduction,has been employed in treating CRF patients with spleenkidney-yang deficiency.The evidence suggests that it offers the following benefits:(1)improvement in white blood cell and neutrophil counts,(2)alleviation of Piper score and fatigue symptoms,(3)enhancement of TCM symptom score,(4)amelioration of Card score,(5)enhancement of sleep quality in CRF patients with spleen-kidney-yang deficiency,which indirectly improves fatigue symptoms,and(6)absence of significant adverse effects.In conclusion,this treatment approach is both safe and effective,making it suitable for wider clinical adoption. |
