| Objective:Through data mining and analysis of medical records,the medication experience,medication rules and treatment ideas of Professor Xue Hanrong in the treatment of patients with pulmonary nodules(Yang deficiency phlegm type)with modified Yanghe decoction were summarized,providing new ideas for the treatment of pulmonary nodules.Method:The medical records of patients with pulmonary nodules(Yang-deficiency-phlegm)in the respiratory Department of Affiliated Hospital of Jiangxi University of Traditional Chinese Medicine were collected,sorted into tables and recorded into the TCM inheritance assistance platform(V2.5)(the clinical collection function of the platform was used to record the patient’s medical records one by one).The input patient information and prescription drugs were analyzed(using data analysis,statistical reports and other functions in the platform),and combined with the follow-up experience and the study of lung nodule related literature,the author analyzed and summarized Professor Xue Hanrong’s medication,prescription rules and treatment ideas for the treatment of lung nodule(Yang-deficiency-phlegm).The medical records of patients with “Medical record analysis” module of the TCM inheritance platform was used to make statistics on the symptoms of patients,and the data were exported and sorted in descending order.A total of 69 symptoms were obtained,among which 23 appeared more than 10 times.Including fear of cold,cough,dry mouth,sweating,loose stool,chest tightness,throat adverse,chest pain,irritability,insomnia,fatigue,abdominal distension,dry throat,throat itching,hiccup,bowel call,bitter mouth,little food,upset,pantothenic acid,nasal congestion,heavy head,god tired.(2)A total of 122 Traditional Chinese Medicine decoction pieces were involved in therapeutic drugs,among which 32 Traditional Chinese Medicine decoction pieces were used more than 10 times.Except for the components of Yanghe Decoction that were used most frequently,the top10 most frequently used drugs were poria cocos,bamboo shavings,immature bitter orange,pericarpium citri reticulatae,Chinese-date,fresh ginger,pinellia,oyster,bupleurum and codonopsis.(3)Different support degrees were set to grasp the whole prescription and the correlation between drugs.The support degree was set as 70% and confidence level as 0.95,including 12 core drugs including antler horn,poria cocos,Rehmannia rehmanniae,Chinese-date,fresh ginger,pericarpium citri reticulatae,bamboo shavings,semen brassicae,immature bitter orange,ephedra and cinnamon.In the setting,the support degree was 50%,the confidence level was 0.8,including deer horn,poria cocos,Chinese-date,baked ginger,pericarpium citri reticulatae,fresh ginger,bamboo shaving,semen brassicae,immature bitter orange,ephedra,cinnamon,honey-fried licorice root,pinellia,a total of 14 Traditional Chinese Medicine decoction pieces.(4)On the basis of core combination analysis,further cluster analysis was carried out to obtain 3 new formulae,namely,new formulae1: Chinese-date,bamboo shavers,fresh ginger,rhodiola rosea,ligusticum and asarum.New recipe 2: Chinese-date,licorice,fresh ginger,pinellia;New Recipe 3: Yam,psoralea,white cardamom,white lentil.Conclusion: The formation of lung nodules is closely related to patients’ age,living environment,eating habits,emotional factors,or the invasion of viscera function due to the perception of external evil;or feelings of internal injury,disorder of Qi;or diet is not good,damage the spleen and stomach,and phlegm and dampness;or body deficiency fatigue,deficiency of positive qi,internal illness.The fundamental cause is the imbalance of Yin and Yang,deficiency damage of the organs,Yang does not transform Qi,Yin condensates into shape,qi stagnation,spittoon,blood stasis,breeding nodules.Therefore,in terms of treatment,the method of warming Yang,eliminating phlegm and disperting tangs,as well as regulating mood,Yang He decoction and Xiaochaihu decoction,Wendan decoction,Chaihuwendan decoction,Guizhifuli decoction,etc.are commonly used in the selection of prescriptions,or different prescriptions are selected according to the patients’ syndrome differentiation,supplemented by mood care,which has a definite clinical effect. |
