| Objective:To compare the efficacy,patient satisfaction,improvement of dark circles,lower eyelid wrinkles,complications and secondary filling of granular fat and SVF-gel(Stromal Vascular Fraction-gel)in the treatment of mild to moderate tear trough deformity,and to analyze the advantages and disadvantages of the two methods,so as to provide reference for clinical application.Methods:From September 2020 to September 2021,40 patients with mild to moderate tear trough deformity in the Department of Plastic and Aesthetic Surgery,the Affiliated Hospital of Qingdao University were randomly divided into two groups.The control group(n = 20)was treated with granular fat injection.The experimental group(n = 20)was treated with SVF-gel injection.The tear trough deformity was graded according to Barton classification before and after operation.The effective rate and patient satisfaction(very dissatisfied 1 point,dissatisfied 2 points,generally satisfied 3 points,satisfied 4 points,very satisfied 5 points)were compared between the two groups at 1 month,3 months and 6 months after operation.Dark circle was evaluated according to the lower eyelid pigmentation score of the Tear Trough Rating Scale(TTRS),and the lower eyelid wrinkles were evaluated according to Fitzpatrick wrinkle score.Before and 6 months after operation,the dark circle and lower eyelid wrinkles of the two groups were scored,and the postoperative score decreased by ≥ 1was defined as improvement.At 6 months after operation,the improvement rate of dark circle and lower eyelid wrinkles were compared between the two groups.The incidence of complications and secondary filling rate were recorded during the follow-up period.To comprehensively analyze the aesthetic effect of granular fat and SVF-gel in improving tear trough deformity.SPSS 26.0 statistical software was used for statistical analysis.Results:There was no significant difference in age,BMI,left filling volume,right filling volume,preoperative Barton’s grade,preoperative dark circle score and preoperative Fitzpatrick score between the control group and the experimental group(P > 0.05).There was no significant difference in the effective rate between the control group and the experimental group at 1month(90% vs.95%)and 3 months(70% vs.90%)after operation(P > 0.05),but there was significant difference in the effective rate at 6 months(50% vs.85%)after operation(P <0.05).The satisfaction rating of the experimental group was higher than that of the control group at 1 month(13.00 ± 3.20 vs.16.50 ± 2.48),3 months(11.75 ± 3.34 vs.15.80 ±2.73)and 6 months(11.65 ± 3.25 vs.15.40 ± 2.33)after operation(P < 0.001).Compared with1 month after operation,the satisfaction rating of the two groups at 3 months(P = 0.008)and6 months(P < 0.001)after operation decreased,and there was no significant difference in satisfaction rating at 3 months and 6 months after operation(P = 0.115).There were significant differences in complication rate(60% vs.20%)and secondary filling rate(35% vs.5%)between the control group and the experimental group(P < 0.05).At 6 months after surgery,there was no significant difference in the improvement rate of dark circle(30% vs.55%)and lower eyelid wrinkles(20% vs.35%)between the control group and the experimental group(P > 0.05).Compared with pre-operation,the dark circle score(1.81 ±0.59 vs.1.46 ± 0.51)and Fitzpatrick score(2.55 ± 1.05 vs.2.35 ± 0.93)in the control group improved at 6 months after operation,the dark circle score(2.00 ± 0.59 vs.1.48 ± 0.71)and Fitzpatrick score(2.65 ± 1.18 vs.2.30 ± 0.98)in the experimental group improved at 6months after operation.Conclusions:Compared with granular fat,the effect of SVF-gel is more durable,the overall satisfaction of patients is higher,and the incidence of complications and secondary filling rate of SVF-gel group are lower.Therefore,SVF-gel is a better filling material,which is worthy of clinical application and promotion. |
