| Objective:(1)To investigate the needs of wound therapists for home care management applets.(2)Develop a scientific home care management program based on the survey results.(3)Verify the effect of the small program on improving self-management,nurse-patient relationship and health education quality,provide effective home care management tools for therapists,and lay the foundation for the formation of scientific chronic wound home care management model in the future.Methods:This study mainly consists of three parts.(1)From January to March 2022,a total of 168 wound therapists across the country were selected as the research objects by the convenient sampling method.The self-designed demand questionnaire for mobile medical applications for home care management of chronic wounds was used to investigate and analyze the current demand status of therapists for small programs.(2)From April to June 2022,group discussion method and Delphi method were adopted.Based on the survey results,the team initially drew up the module content of the home care management small program,developed a letter questionnaire,selected consultation experts,and conducted two rounds of letter consultation with 20 experts.The development team used JS,JSON,WXML,WXSS and other programming languages to design and develop small programs.(3)From July to October 2022,33 patients with chronic wounds admitted to a Classⅲ Grade A hospital in Shanxi Province were selected as the research objects by the method of quasi-experimental research.The intervention was a home care management plan with the small program as the carrier.Self-management Scale for Patients with Chronic Wounds,Nurse-Patient Relationship Trust Scale and Health Education Satisfaction Questionnaire were used to evaluate the effect of home care before and after intervention.Results:(1)The total average score of 168 wound therapists’ demand for mobile medical applications for home care management was(4.38 ± 0.42),and the top three module scores were: The score of dressing change management was(4.56±0.46),the score of consultation service was(4.55±0.46),and the score of health education was(4.38±0.49).The results of correlation analysis showed that the age of therapists was negatively correlated with the application demand(r=-0.229,P < 0.05).(2)The effective recovery rates of questionnaires in two rounds of consultation among 20 experts were 100.0% and 90.0%,and the expert authority coefficients(Cr)were 0.8725 and 0.875,respectively.The Kendall coefficient was statistically significant(P < 0.05).Finally,the module content of the home care management small program including 3 first-level modules,11 second-level modules and 40 third-level modules was determined.(3)33 patients with chronic wounds completed the intervention study.(1)The total average score and the average score of five dimensions of self-management were significantly improved before and after the intervention,and the differences were statistically significant(P < 0.05).(2)The total average score of nurse-patient relationship trust and the average score of two dimensions were significantly improved before and after the intervention,and the differences were statistically significant(P < 0.05).(3)The total average score and the average score of two dimensions of satisfaction with health education quality before and after intervention were significantly improved,and the differences were statistically significant(P < 0.05).Conclusion:(1)Wound therapists had a high demand for mobile medical applications for home care management of chronic wounds;However,therapists of different ages have different needs for the application,and the program should be designed according to the needs of therapists,so as to improve the efficiency and quality of home care management.(2)The module content of the home care management small program based on Delphi method in this study is scientific and reasonable,which can be applied in clinical practice.(3)The mini program can effectively improve patients’ self-management ability,nurse-patient trust and quality of health education,and call for more medical staff to participate in the construction of mobile medical applications for home care management. |
