ObjectiveThe purpose of this study is to investigate the current situation of presenteeism,professional identity and organizational climate of emergency department nurses,to explore the influencing factors of presenteeism of emergency department nurses,and the influence of professional identity and organizational climate on presenteeism,to provide a reference for nursing managers to formulate corresponding countermeasures and reduce the level of presenteeism of nurses in the emergency department,to reduce the loss of health productivity of emergency department nurses and improve the quality of nursing work,to ensure the safety of patients and the physical and mental health of nurses and promote the stable and healthy development of the emergency nursing team.MethodsA convenience sampling method was used to survey 342 emergency department nurses in seven tertiary care hospitals in Shandong Province and Hebei Province as the study population using the general information questionnaire,the Chinese version of the Stanford Presenteeism Scale,the Nurses’ Professional Identity Rating Scale,and the Nursing Organizational Climate Perception Scale.Excel 2010 was used to establish the database,and SPSS 26.0 software was used for statistical analysis,and frequency(n)and percentage(%)were used to statistically describe the demographic information and the current status of each variable.The scores of presenteeism,professional identity,and organizational climate of emergency department nurses were described by the mean ± standard deviation;independent sample t-test or one-way ANOVA was used to analyze the influence of general demographic data on latent absenteeism,professional identity and organizational climate of emergency department nurses;Pearson Correlation Analysis was used to analyze the correlation between presenteeism,professional identity and organizational climate of nurses in the emergency department,and multiple linear regression model was constructed to explore the influence of professional identity and organizational climate on presenteeism;Structural equation modeling was constructed to analyze whether there was a mediating effect among presenteeism,professional identity and organizational climate.Results1.The total presenteeism score of nurses in the emergency department was16.01±4.63,with a mean entry score of 2.67±0.77.Using the median of the total presenteeism score of 16 as the cut-off point,the group was divided into low and high presenteeism groups,with 179(52.3%)in the low presenteeism group and 163(47.7%)in the high presenteeism group,and nurses with different health statuses in the presenteeism score.The difference was statistically significant(P<0.05).2.The total score of professional identity was 112.03±21.29 with a mean score of3.73±0.71.The difference in the total mean score of professional identity was statistically significant(P<0.05)among emergency department nurses with different health statuses,and the difference in the total mean score of professional social skills was statistically significant(P<0.05)among emergency department nurses with different genders.3.The total score of organizational climates was 116.16±20.99,with a mean score of 3.14±0.57,and the difference in the total mean organizational climate score of emergency department nurses with different health statuses was statistically significant(P<0.05).4.There was a two-by-two correlation between presenteeism,professional identity and organizational climate among emergency department nurses,with professional identity negatively correlated with presenteeism(r=-0.383,P<0.01),organizational climate negatively correlated with presenteeism(r=-0.341,P<0.01),and professional identity positively correlated with organizational climate(r=0.825,P<0.01).5.The results of multiple stepwise regression showed that there were three variables in the regression equation: occupational social skills,occupational frustration coping and occupational self-reflection(P< 0.05);To analyze the effect of organizational climate on recessive absenteeism scores of nurses in the emergency department,three variables entered the regression equation: resources and guarantee,group behavior and management support(P < 0.05).6.Structural equation modeling revealed that the professional identity of emergency department nurses mediated the relationship between perceived nursing organizational climate and presenteeism,with an indirect effect of 89.2% of the total effect.Conclusion1.This study shows that the presenteeism of emergency department nurses is at a high level and that presenteeism is prevalent among emergency department nurses;the professional identity of emergency department nurses is at a moderate to a high level;and the organizational climate of emergency department nurses is at a moderate level.2.Among the general demographic variables’ health status is an influencing factor for presenteeism among emergency department nurses,health status and gender are influencing factors for professional identity,and health status is an influencing factor for the organizational climate.3.There is a significant correlation between presenteeism,professional identity and organizational climate among emergency department nurses,and both professional identity and organizational climate are predictive of presenteeism.4.The professional identity of emergency nurses plays a mediating role between organizational climate and presenteeism.
