Objective:In recent years,the incidence rate of dizziness/vertigo has gradually increased,and most of them are manifested as paroxysmal vestibular syndrome.Relevant knowledge involves ear,nose and throat,mental health,emergency,heart,psychology and other disciplines.At present,most hospitals in China do not set up specialized outpatient clinics,and patients are difficult to diagnose and are prone to missed diagnosis and misdiagnosis.We found that several common paroxysmal vestibular syndrome diseases can be preliminarily identified by inquiring about the medical history.Therefore,in order to screen patients with recurrent dizziness/vertigo more effectively,we developed a screening questionnaire for paroxysmal vestibular syndrome diseases,which forms a preliminary diagnosis through the questionnaire,improves the efficiency of treatment,and provides a convenient and effective tool for primary medical institutions to diagnose and treat patients with dizziness/vertigo.Methods:By consulting the literature,combining the clinical characteristics of paroxysmal vestibular syndrome,and using expert group meetings to seek expert opinions,the content of the questionnaire for screening paroxysmal vestibular syndrome diseases was determined.By using convenient sampling,we prospectively selected the patients who complained of dizziness/vertigo in the outpatient department of Otolaryngology Head and Neck Surgery from January 2022 to September 2022 in a third-class hospital in Shanxi Province for questionnaire screening.A total of 200 patients were included before and after the screening.After the questionnaire screening,the dizziness/vertigo experts will conduct a consultation,and take the corresponding vestibular test as the gold standard to determine the final diagnosis of the patient.Those who cannot be diagnosed will be excluded.This study is divided into two steps.The first step is to conduct factor analysis and adjust the questionnaire entries;The second step is to evaluate the effectiveness of the questionnaire diagnosis.Results:Finally,23 items were determined: 1.Sometimes I feel dizzy and sometimes I have a headache;2.My dizziness often turns around;3.My dizziness occurs or aggravates when I lie down and get up/move my head quickly/turn over in bed,or occurs after I hit my head(one item can be satisfied);4.I have dizziness many times every day,and each dizziness lasts only a few seconds;5.I am prone to dizziness when defecating hard;6.When I feel dizzy,I will have tinnitus on both sides;7.My dizziness or dizziness lasts less than 5 minutes each time;8.I feel dizzy more than 10 times or more every day;9.My dizziness and headache are often accompanied;10.Unilateral tinnitus or increased tinnitus may occur before or during the onset of dizziness;11.When I walk and turn my head normally,I will not feel like spinning around;12.When I move my head,I will have a brief dizziness;13.I am prone to dizziness when facing noise;14.Before or during the dizziness attack,my hearing in one ear decreased significantly,but after the dizziness improved,my hearing loss will gradually improve;15.When I feel dizzy/dizzy,there is no ear tightness or hearing loss;16.Have a history of migraine before;17.My tinnitus will reduce or disappear after dizziness improves;18.My dizziness/vertigo occurs irregularly;19.My dizziness is often accompanied by nausea and even vomiting;20.Strong light will aggravate my dizziness or make me more uncomfortable;21.Strong voice will aggravate my dizziness or make me more uncomfortable;22.My family also has a history of migraine or repeated dizziness;23.I have carsickness.The overall prediction ability of the questionnaire is 67.1%,the prediction ability of VP and SSCD is 100%,the prediction ability of MD is good,up to 73.7%,the prediction ability of VM is good,up to 66.7%,and the prediction ability of BPPV is fair,55.6%.The research shows that the questionnaire has good screening ability for EVS diseases.Conclusion:The questionnaire has a good ability to predict disease,and can be used to screen EVS patients and provide patients with preliminary diagnosis and subsequent treatment plans.
