| Objective:To improve the quality of standardized residency training(hereafter referred to as‘SRT’)in Shanxi Province,based on the SRT model of‘clinic tutors-trainees interaction and mutual learning’,the quality management theoretical model PDCA cycle is introduced into SRT management.Investigate the effects of PDCA cycle on optimizing the clinic tutors of SRT and improving the quality of SRT.Method:Using a structured questionnaire survey.In 2020,a survey was conducted on the clinic tutors of the SRT bases in Shanxi Province to understand their status and the SRT situations in Shanxi Province.The PDCA cycle model was introduced into the management of the clinic tutors of SRT in Shanxi Province,and the‘training-assessment-access-clearance’mechanism was formulated according to the problems found,and implemented.In 2021,a structured questionnaire was once again used to investigate the clinic tutors of SRT in Shanxi Province.In 2020 and 2021,2340 and 2036 valid questionnaires were collected respectively,and the respondents covered all of the clinic tutors of SRT in the province that year.By comparing the structure of clinic tutors surveyed in 2021 and 2020,their cognition of SRT work,the passing rate of SRT assessment,training effects and the test of the professional level of SRT bases,the effectiveness of the PDCA cycle management model implemented by clinic tutors of SRT was evaluated.The database was established with SPSS19.0,and the enumeration data was analyzed descriptively.Optimize the team of clinic tutors of SRT in Shanxi Province,using Visual Studio Code 1.65.0 to render word cloud maps,and explore the problems and difficulties of SRT management.Results:According to the 2020 SRT clinic tutors survey,36.8%are 36~40 years old,49.1%have master’s degrees,and 56.2%have senior professional titles;the number of years of employment is mostly 11~15 years.About 11.1%of the clinic tutors are graduate mentors;84.3%carried out teaching rounds as scheduled,and94.7%thought that teaching rounds,case discussions,small lectures,etc.were relatively standardized;97.8%were able to establish friendly relationships with the trainees;75.3%can achieve hands-off practical teaching;92.3%thought that the rotation arrangement was reasonable;98.1%can guide the trainees’practical operation and learning;93.2%gave feedback on the writing of trainees’case writing;91.2%recognized the necessity of SRT;clinic tutors are not most eager to increase subsidies and benefits,but their first priority is to provide them with training opportunities to improve their own teaching levels,accounting for 84.8%;followed by the preference in title evaluation and promotion,accounting for 76.0%;then came the improvement in subsidies,accounting for 72.4%.The top 4 words in the words cloud map results were‘trainees’448 times;‘Time’269 times;‘worked’246 times;‘Teaching’230 times.According to the 2021 SRT clinic tutors survey,814(40.0%)of the 2,036interviewed clinic tutors said that they were very willing to teach;1160(57.0%)said they were relatively willing to teach;57(2.8%)said they were less willing to teach;only 5(0.2%)were very reluctant to teach;The top 4 words in the words cloud map are‘trainees’370 times;‘Time’242 times;‘worked’182 times;‘Positive’157 times.After the implementation of PDCA cycle model for clinic tutors,the proportion of them who with master’s and doctoral degrees increased from 55.9%to 63.0%in 2021 compared with 2020(χ~2=23.499,P<0.001),and the proportion of senior professional titles increased from 56.2%to 64.6(χ~2=31.772,P<0.001).The proportion of clinic tutors who participated in national training increased from29.4%to 35.8%(χ~2=20.086,P<0.001);The proportion of clinic tutors who did not think it was necessary to carry out the SRT decreased from 7.9%to 1.7%(χ~2=100.61,P<0.001);the proportion of clinical practice teaching methods‘letting go and looking away’increased from 5.5%in 2020 to 13.0%in 2021(χ~2=85.943,P<0.001);the proportion of trainees who believed that their mastery of teaching content was not standardized decreased from 5.3%in 2020 to 4.0%in 2021(χ~2=158.414,P<0.001);the pass rate of residency trainees assessment has increased significantly,and in the overall ranking in China,it rose 2 places in2019-2020,ranking 20th in the country,and increased 6 places in 2020-2021,ranking 14th in the country;The professional level test results of Shanxi Province’s SRT bases have achieved a leap from scratch to excellence in the top 100 national rankings.The overall satisfaction of clinic tutors in Shanxi Province with the management of the SRT bases was more than 87.5%.Conclusion:The SRT clinic tutors management based on the PDCA cycle model plays an important role in strengthening clinic tutors’training,optimizing the structure of the staff team,improving the quality of SRT,and improving the professional level of SRT bases.It is recommended to continue to explore the application of PDCA cycle in SRT management,which is of great significance to improve the effect and quality of the SRT. |
