| Objective: To investigate the effect of tendon manipulation on the histopathology of intervertebral disc degeneration-vertebral disc fat infiltration in rabbits by observing the effects of tendon manipulation on the histopathology of intervertebral disc degeneration-vertebral disc fat infiltration,the levels of serum adipokines and inflammatory factors,the expression of Wnt/β-catenin pathway and transcription factors PPARγ and CEBPB in intervertebral disc degeneration-vertebral disc fat infiltration.Methods: Six rabbits were randomly selected from 18 clean grade New Zealand rabbits as the control group,while the remaining 12 rabbits were uniformly induced with a percutaneous fibrous ring puncture technique to induce intervertebral disc degeneration-fatty infiltration of the erector spinae muscle.The three groups were fed and watered normally after modelling.The treatment group was treated 1 month after modelling with tendon manipulation focusing on the paravertebral erector spinae and multifidus muscles of the lumbar back,once/every other day,for 6 weeks,and then the three groups were simultaneously executed and the material taken.The L4/5 segmental erector spinae muscles and intervertebral discs were collected,and serum levels of IL-1,TNF-α,LEP and ADPN were measured by ELISA.Histopathological changes in the erector spinae muscles and intervertebral discs of each group were observed by HE staining under a microscope,and the expression of Wnt10 b,β-catenin protein,PPARγ and CEBPB transcription factors in the Wnt/β-catenin pathway were detected by immunohistochemistry,CEBPB transcription factor expression in the Wnt/β-catenin pathway.Statistical SPSS software was also used for experimental analysis and Graph Pad Prism was used to produce graphical analysis.Results:(1)ELISA results showed that: compared with the control group TNF-α and LEP serum levels were significantly higher in the model group(P<0.05)and significantly lower in the treated group compared with the model group(P<0.05);compared with the control group IL-1 serum levels were significantly higher in the model group(P<0.01)and significantly lower in the treated group compared with the model group(P<0.05)The serum level of ADNP was significantly lower in the model group compared with the control group(P<0.01)and significantly higher in the treated group compared with the model group(P<0.05).(2)HE staining results showed that the muscle pathology of the erector spinae muscle,compared with the control group,the model group had fewer myofibroblasts,disorganized muscle fiber arrangement,increased muscle bundle interval,clusters of adipocytes within and between muscle bundles,increased in number and volume,and the muscle bundle interval in the tendon manipulation treatment group was smaller than the model group,and the number and volume of adipocytes were also smaller than the model group.Compared with the control group,the elliptical shape of the nucleus pulposus in the model group was significantly deformed,the volume was smaller,the gel-like nucleus pulposus content was significantly reduced,the tissue arrangement was disorganized,the boundary with the inner fibrous ring was blurred,and there were obvious signs of fissures;compared with the model group,the quantity of gel-like nucleus pulposus content in the tendon manipulation group was significantly increased,the elliptical shape of the nucleus pulposus was significantly restored,and the fissures between the inner fibrous ring and the nucleus pulposus were significantly less,and the boundary became clearer.The boundaries became more clearly defined.(3)Immunohistochemical results showed that the expression levels of Wnt10 b and β-catenin in the model group were significantly lower than those in the control group(P<0.05),and the expression levels of PPARγ and CEBPB in the model group were significantly higher than those in the control group(P<0.05).The expression levels of PPARγ and CEBPB in the model group were significantly higher than those in the control group(P<0.05),and the tendon manipulation significantly reduced the expression of PPARγ and CEBPB in the erector spinae muscle compared with that in the model group(P<0.05).Conclusion: Tendon manipulation can benignly regulate the expression level of inflammatory factors and adipokines,improve the intervertebral disc microenvironment,and regulate the fat infiltration of erector spinal muscle by activating the Wnt/β-catenin pathway,improve muscle mass,restore muscle physiological structure and function,increase spinal stability,and then play a role in preventing and treating intervertebral disc degeneration. |
