Objective:This study will investigate the diagnostic value of the combination of carbohydrate antigen 125(CA125),neutrophil to lymphocyte ratio(NLR)and fibrinogen(FIB)for epithelial ovarian cancer(EOC),and provide a reference for improving the diagnosis of EOC.Methods:Retrospective analysis of clinical data of 158 patients with EOC and172 patients with benign ovarian tumors who visited the Department of Gynecology of the Hebei General Hospital and underwent surgery from January 2018 to March 2022.The differences between preoperative clinical indicators in EOC and benign ovarian tumors were compared by Mann Whitney U test,and the differences between preoperative clinical indicators and the pathological and clinical general characteristics of EOC were compared by Mann Whitney U test;univariate logistic regression and multivariate logistic regression analyses were performed for indicators that differed between the two groups for EOC and benign ovarian tumors,and the diagnostic value of CA125,NLR,and fibrinogen alone and in combination tests for EOC was analyzed using the receiver operating characteristic curve(ROC).Results:1.The EOC group displayed significantly higher age,neutrophil count,blood platelet count,CA125,human epididymis protein 4(HE4),NLR,platelet to lymphocyte ratio(PLR)and fibrinogen than the benign ovarian tumor group,with a statistically significant difference(P < 0.05);the lymphocyte count in the benign ovarian tumor group was higher than that in the EOC group,and the difference was statistically significant(P <0.05).2.CA125 and NLR were significantly correlated with age(≥50 years),stage,grade,pathological pattern,amount of abdominal water(≥500 ml),presence of lymph node metastasis,and whether cancer cells were seen in the omentum,respectively(P<0.05),and fibrinogen was significantly correlated with age(≥50 years),pathological stage,tissue differentiation,amount of abdominal water(≥500 ml),presence of lymph node metastasis,and whether cancer cells were seen in the omentum cancer cells were significantly correlated(P<0.05).3.Logistic regression model analysis showed that elevated CA125,NLR and fibrinogen were all independent risk factors for EOC.4.Results of the ROC curve:When benign ovarian tumors were used as controls,the area under the ROC curve(AUC)of CA125,NLR and fibrinogen in the diagnosis of EOC were 0.881,0.804 and 0.830,respectively;the optimal threshold values of CA125,NLR and fibrinogen were 35.720 U/ml,2.081,and 3.070g/L,with sensitivities of 78.5%,77.2%and 72.2%,and specificities of 86.6%,70.3% and 84.9%,respectively.5.Results of the ROC curve:The combination of CA125、NLR and fibrinogen had the largest area under the curve in identifying benign and malignant ovarian tumors with 0.926,sensitivity 76.6% and specificity94.2%,and the value of the combination of the three was better than CA125,CA125 with NLR and CA125 with fibrinogen in diagnosing EOC.Conclusion:1.The differences in CA125,NLR and fibrinogen in the EOC group and the group with benign ovarian tumors,suggesting that the three have some auxiliary value in identifying benign and malignant ovarian tumors.2.Higher values of CA125 and NLR in the serum of EOC patients suggest a greater likelihood of plasmacytic histology.3.The higher the CA125 and NLR values in the serum of EOC patients,the more likely it is that the histology is plasmacytic;the higher the CA125,NLR and fibrinogen values,the more advanced the stage,the more combined abdominal water,and the more likely it is that the lymph nodes and large omental metastases are metastatic;it is expected that CA125,NLR and fibrinogen will be used as indicators to assess the change of EOC disease.4.Elevated CA125,NLR and fibrinogen are all independent risk factors for EOC;the value of combined CA125,NLR and fibrinogen in the diagnosis of EOC is better than CA125,CA125 and NLR,CA125 and fibrinogen,and the clinical operation is simple and safe,which has some auxiliary value in the diagnosis of EOC.
