Purpose:In this study we established the myofascial pain syndromes(MPS)rats model by strinking on fascia musculares and eccentric exercise,and used traditional milli-acupuncture and mild moxibustion on the myofascial trigger points in rats,and by behavioral observation of pain,electrophysiological testing,histological observation and biochemical indexes related to detection to evaluate the inactivating effect of acupuncture and moxibustion on the myofascial trigger points,explore theirs mechanisms underlying improvement of MPS,in order to provide experimental basis for clinical acupuncture treatment of MPS.Material and method:After a week of adaptive feeding,a total of 32 male SPF rats were stratified by whether they were stratified with the movement of the runway,and then randomly divided into control group and MPS group.The MPS model was established by strinking on fascia musculares and eccentric exercise.After the model was successfully prepared,rats were then divided into a model group,a moxibustion group and an acupuncture group according to a random number table method.Rats in the moxibustion group and the acupuncture group were treated at myofascial trigger point for 15 min,once daily for 7 days.Rats in the model group were fixed with the same method as treatment groups without other intervention.Rats in the control group were fed normally without any experimental intervention.During the experiment,the general states of rats in each group were observed,and the weight changes of rats were recorded.The thermal withdrawal latency(TWL)of rats was measured with a hot stabbing instrument.The spontaneous electrical activities of rats was tested by electromyography.The pathological changes of the rat medial femoris muscle were observed after H.E.staining.the levels of interleukin-8(IL-8)and prostaglandin E2(PGE2)in abdominal aortic blood were detected by enzyme-linked immunosorbent assay(ELISA).The expressions of microglia marker(OX-42)and BDNF in the spinal dorsal horn were detected by immunohistochemistry and Western blot,separately.the m RNA content of OX-42 in rat spinal cord tissue by RT-PCR.Results:1.Rats in general: The control group rats had clean and smooth fur,no abnormality in diet and drinking water,normal activity,normal stool,good mental state,gentle temperament,and two eyes;the rats in the model group had dirty fur,yellow and dull skin color,reduced food intake,increased water intake,increased urination,individual diarrhea phenomenon,perianal filth,larger amount of activity,like to close their eyes,often bury their heads in the abdomen.The rats in the acupuncture group and the moxibustion group had rough fur after the end of treatment,had normal diet and drinking water,less activity,reduced exploration behavior,normal stool,good mental state,and partial closure of individual eyelids.Before molding,there was no significant difference in body mass between the rat groups(P>0.05).After molding,the body quality of the model group,acupuncture group and moxibustion group was significantly reduced compared with the control group(P<0.01).After treatment,compared with after molding,the mass of the moxibustion group and acupuncture group increased(P<0.05 or P<0.01).2.TWL changes: Before molding,there was no significant difference in TWL between the four groups(P>0.05).After molding,the TWL of the model group,acupuncture group and moxibustion group was significantly down-regulated(P<0.01),and was significantly decreased in contrast to that of the control group(P<0.01),indicating that MPS molding can reduce the TWL of rats.After treatment and compared with the model group,the TWL of the acupuncture group and the moxibustion group was significantly increased(P<0.01).After treatment,there was no significant difference between the acupuncture group and moxibustion group(P>0.05).3.SEA changes: after treatment,there was still no spontaneous electrical activitie in the control group;regular and frequent spontaneous electrical activities were visible in the model group within 10 minutes.In the moxibustion group and acupuncture group,only a few rats had occasional spontaneous electrical activities,and the self-potential frequency was significantly lower than that in the model group.4.Muscle tissue pathological changes: Slice horizontally: the size of the muscle tissue in the control group was uniform and equal,the arrangement was regular,the cells were regular polygonal,and the nucleus located under the muscle membrane was clearly visible at the edge of each cell.The muscle tissues in the model group vary in size,the cells were irregularly deformed,the nuclei moved inward,and there were multiple enlarged,circular,deeply stained,densely packed contracture nodules.Muscle tissue cells in the moxibustion group were relatively regular,and there was nucleus internal migration.The muscle tissue in the acupuncture group was arranged in disorder,and a small number of nuclei were displaced inward.Slice vertically : the muscle fibers in the control group were uniformly thick and thin,tightly arranged and regular,and the nuclei were mostly located under the myofiber membrane.The muscle fibers in the model group were ruptured,varying in size and thickness,irregular arrangement,a large number of inflammatory cell infiltrates and edema,and connective structural tissues are newly adhesions.The muscle fibers in the moxibustion group were relatively uniform in thickness,neatly arranged,and the morphology was close to normal,and a small amount of inflammatory cell infiltration was reduced.The muscle fibers in the acupuncture group were more uniform in thickness,arranged more neatly and orderly,the muscle fiber rupture was significantly improved,and the fibrotic adhesions and inflammatory cell infiltrates were less.5.IHC results showed that the control group had light brown scattered microglia(resting state,small and slendcell body,dendritic),and the model group had some degrees of dark brown microglia activation(hypertrophy cell body,less neurite).Compared with the control group,the expression of OX-42 in the spinal dorsal horn of rats in the model group was significantly increased(P<0.01).Compared with the model group,the OX-42 of moxibustion group and acupuncture group was significantly reduced(P<0.01),and compared with the moxibustion group,the microglia cells in the acupuncture group were close to normal,and the average absorbance value was reduced(P<0.05).6.Werstern bolt results showed that compared with the control group,the expression of BDNF in the spinal cord of rats in the model group was significantly up-regulated(P<0.01).Compared with the model group,the expression of BDNF of moxibustion group and acupuncture group was significantly down-regulated(P<0.01),and compared with the moxibustion group,the expression of BDNF of acupuncture group was significantly down-regulated(P<0.01).7.RT-PCR results showed that compared with the control group,the expression of OX-42 m RNA of the remaining three groups was increased(P<0.05 or P<0.01).Compared with the model group,the expression of OX-42 m RNA of moxibustion group and acupuncture group was reduced(P<0.05 or P<0.01).The expression of OX-42 m RNA of acupuncture group was less than moxibustion group(P<0.05).Conclusion:1.Acupuncture and moxibustion can improve the pain behavior of MPS rats,increase the TWL,promote the recovery of myohistitis injury,reduce the frequency of self-generating activities at the myofascial trigger points,and play an inactivating role in the myofascial trigger points.2.The Anti-inflammatory and analgesic effect of acupuncture and moxibustion on MPS may be related to reduce the serum IL-8 and PGE2 levels and inhibit the activation of spinal cord microglia OX-42 and BDNF expression.Moxibustion is better at reducing inflammation than acupuncture,and acupuncture is better at inhibiting central sensitization than moxibustion.
