OBJECTIVE:1.To observe the clinical efficacy and safety of fire needle combined with filiform needle in the treatment of tension type headache with pericranial tenderness.2.To observe the changes of pericranial muscle tension in TTH patients after treatment,and to evaluate the correlation with the efficacy.METHOD:A prospective randomized controlled trial was conducted to include the subjects in strict accordance with the design criteria,and the clinical observation of 79 patients with frequent and chronic tension-type headache with pericranial muscle tenderness was completed,including 41 cases in the experimental group and 38 cases in the control group.The experimental group was treated with fire needle fast puncture pericranial tenderness combined with filiform needle tranquilizing treatment,the control group was treated with filiform needle tranquilizing treatment,2 times a week,a total of 16 times.The number of headache days,headache duration,VAS,SF-MPQ scale,HIT-6 scale,number of pericranial tender points,pressure pain threshold and SWE value were recorded before treatment,after treatment and follow-up period.By analyzing the changes of various data,the efficacy of fire needle combined with filiform needle in the treatment of TTH was evaluated,and The correlation between headache days and headache degree changes and pericranial muscle tension changes was analyzed.RESULT:(1)Efficacy evaluation:After treatment,the effective rate of the experimental group was 87.80%,and that of the control group was 57.89%.The difference was statistically significant(P<0.05).During the follow-up period,the effective rate of the experimental group was 85.36%,and that of the control group was 50.00%.The difference was statistically significant(P<0.05).(2)The number of days and duration of headache:After 8 weeks of treatment,the number of days and duration of headache in both groups were significantly lower than those before treatment(P<0.05),and the improvement in the experimental group was more significant than that in the control group(P<0.05).(3)Headache degree:After 8 weeks of treatment,the VAS,VRS,PPI and PRI(S)scores of the two groups were significantly lower than those before treatment,and the differences were statistically significant(P<0.05).The experimental group decreased more significantly(P<0.05).(4)Emotion and quality of life:After 8 weeks of treatment,the two groups of patients after treatment and follow-up period,the mental mood and life treatment were significantly improved compared with before treatment,the difference was statistically significant(P<0.05).The improvement of the experimental group was more obvious(P<0.05).(5)Changes of pericranial muscle tension:Through palpation examination of the patient’s pericranial muscles,it was found that the areas with the most frequent tenderness points were the superior trapezius muscle,sternocleidomastoid muscle,superior nuchal line,temporal muscle,and masseter muscl.Among them,the superior trapezius muscle,sternocleidomastoid muscle,superior nuchal line and nuchal plane of occipital region are important distribution areas of pericranial tenderness.After 8 weeks of treatment,the two groups of patients with upper trapezius Young’s modulus values than before treatment decreased,the test group was more obvious,but not statistically significant(P>0.05);The number of pericranial tenderness points in the experimental group was significantly reduced,and the tenderness threshold was significantly increased(P<0.05).There was no significant change in the control group.After treatment,the decrease of headache attack days and the decrease of headache degree were positively correlated with the decrease of tenderness points and the increase of tenderness threshold,and the difference was statistically significant(P<0.05).CONCLUSION:1.For patients with TTH,fire needle puncture pericranial tenderness points combined with filiform needle Anshen compared with simple filiform needle treatment,can effectively reduce the number of days of patients with headache attack,shorten the average duration of headache,effectively reduce the degree of headache,excite the patient’s mental mood,improve the quality of life,safe and stable,has a good long-term effect.2.The areas of pericranial muscle tenderness points in TTH patients were the superior trapezius muscle,sternocleidomastoid muscle,superior nuchal line,temporal muscle and masseter muscle.The superior trapezius muscle,sternocleidomastoid muscle,superior neck line and neck plane of the occipital region are important distribution areas of pericranial tenderness points.3.Fire needle combined with filiform needle therapy can more effectively reduce the number of tenderness points and increase tenderness threshold than simple filiform needle therapy;the decrease of headache days and headache degree were correlated with the decrease of tenderness points and the increase of tenderness threshold.
