| Objective:The incidence of erectile dysfunction(ED)is increasing year by year,which not only affects the quality of life of men and their spouses,but also serves as an early warning signal for other diseases such as cardiovascular disease.In modern medicine,type 5 phosphodiesterase inhibitors are usually the first-line treatment plan for ED.Due to the uncertainty of their long-term efficacy and related side effects,patients sometimes do not accept them,requiring additional treatment drug choices.Traditional Chinese medicine,with its unique characteristics of multiple components and multiple targets,has unique advantages in the treatment of ED and is worth further development.However,currently,empirical treatment is still the main approach in clinical practice.Professor Li Yueqing is a renowned traditional Chinese medicine practitioner in China who has been engaged in male medicine for decades and has rich experience in treating this disease,Therefore,this study aims to use the "Ancient and Modern Medical Record Cloud Platform(V2.3.5)" to explore and analyze the medication patterns of Professor Li Yueqing’s treatment of ED,provide relevant ideas for clinical treatment of this disease,summarize core prescriptions,and use network pharmacology methods to study the potential targets of Professor Li Yueqing’s treatment of ED,preliminarily explore its mechanism of action,and lay the foundation for subsequent experimental research.Method:Collect traditional Chinese medicine prescriptions for the treatment of ED issued by Professor Li Yueqing at the Men’s Special Clinic of Dongzhimen Hospital,Beijing University of Traditional Chinese Medicine from January 2021 to June 2022.Enter information such as number,diagnosis frequency,traditional Chinese medicine syndrome,and composition of traditional Chinese medicine,and double check and cross check to ensure accuracy.If there are any questions,they need to be discussed and resolved.After the verification is completed,establish the final database.Add the database to the analysis pool,use platform algorithms to analyze traditional Chinese medicine syndromes,frequency,attributes,and other contents,conduct data mining,explore Professor Li’s medication patterns for treating ED,and obtain the core prescription for Professor Li’s treatment of ED based on complex network analysis.Use the TCMSP database to retrieve the chemical composition of each drug in the core prescription separately.Using the PubChem platform database of organic small molecule bioactivity,combined with the SwissTargetPrediction platform,to screen potential targets for traditional Chinese medicine compounds.Retrieve and screen targets related to ED through the Genecards database.Import the obtained traditional Chinese medicine targets and ED disease targets to Venn website,screen and record the common targets of traditional Chinese medicine and diseases.Import common targets into the String network platform and combine them with Cytoscape software to obtain PPI network diagrams and potential key targets for prescription therapy of ED.Finally,the common targets were imported into the DAVID database for GO and KEGG enrichment analysis to analyze their possible mechanisms.Result:1.This study included a total of 501 prescriptions,involving a total of 212 traditional Chinese medicines,and the frequency of use of each drug was 8136 times.Professor Li Yueqing’s commonly used drugs for treating ED include Achyranthes Bidentata,White Peony,Morinda officinalis,Bupleurum chinense,Poria cocos,Fructus Schisandrae,Salvia miltiorrhiza,goji berry,Fructus Aurantii,Radix Polygalae,Fructus Rhodiolae,Poria,Epimedium,Acorus tatarinowii,Astragalus membranaceus,Cloves,Hypericum perforatum,Fructus Ziziphi spinosum,Cistanche deserticola,etc.The drugs are mostly mild in temperature,sweet in taste,and mostly belong to the liver and kidney meridians,with the main focus on"tonifying the kidney and soothing the liver,promoting blood circulation and yang",and are compatible with dampness,regulating qi Medications such as nourishing the mind and calming the mind.The final core prescription for treating ED by Professor Li Yueqing was determined to consist of the following drugs:Bupleurum chinense,White peony,Fructus Aurantii,Cuscuta chinensis,goji berry,Schisandra chinensis,Morinda officinalis,Rhodiola officinalis,Polygala tenuifolia,Centipede,Salvia miltiorrhiza,Achyranthes bidentata,and Poria cocos.Further clinical validation and optimization are needed.2.A total of 196 potential core compounds,quercetin,kaempferol,stigmasterol βSitosterol and other ingredients may be important in the core prescription.By screening the active ingredients,863 corresponding targets of the compounds were predicted.By constructing the Venn map and intersecting with the top 500 targets of the disease,116 common targets of traditional Chinese medicine and disease could be obtained.TNF,AKT1,GAPDH,VEGFA,CASP3,EGFR,STAT3,HIF1A,ESR1,MMP9 and other key targets for the treatment of ED by the core prescription.GO analysis and KEGG analysis found that the core prescription involved 116 biological processes,75 cell components,131 molecular functions,138 signal pathways,including calcium signal pathway,PI3K-AKT signal pathway,HIF-1 signal pathway,MAPK signal pathway,Ras signal pathway,etc.Conclusion:1.The pathogenesis of ED is mainly blood stasis,liver stagnation and kidney deficiency,and the disease location is mainly in liver and kidney;Professor Li Yueqing’s clinical medication for the treatment of this disease"is based on tonifying the kidney and soothing the liver,activating the blood and invigorating the yang".Most of the medication belongs to the liver and kidney meridians,and most of the people prefer to use the drugs with a peaceful nature.2.The core prescription composition and dosage are as follows:Bupleurum chinense 10g;White peony 12g;Fructus Aurantii 10g;Dodder seed 12g;Chinese wolfberry 15g;Schisandra chinensis 12g;Morinda officinalis 15g;Rhodiola 15g;Polygala l0g;Centipede 4g;Salvia miltiorrhiza 15g;Achyranthes bidentata 12g;Poria cocos 15g.3.The treatment of ED with core prescription may mainly involve anti-oxidation,anti-apoptosis,anti-fibrosis,improvement of vascular endothelial function,regulation of ion channels and metabolism through the joint action of multiple components and multiple targets. |
