Purpose:Patients with rheumatoid arthritis(rheumatoid arthritis,RA)combined with atherosclerotic cardiovascular disease(atherosclerotic cardiovascular diseases,ASCVD)were collected, and the distribution characteristics of TCM syndrome types and their correlation with cardiovascular risk factors were analyzed to understand the essence of the syndrome type and provide reference for RA patients with ASCVD.Material and method:According to the row standard collection in November 2019-January 2023 in Liaoning university of Chinese medicine affiliated hospital rheumatism hospital RA with ASCVD patients, collect patients general condition,TCM syndrome, rheumatoid arthritis patients condition evaluation(DAS28 score),laboratory indicators, using SPSS 24.0 software to explore the correlation between TCM syndrome and the clinical data.Results:1.A total of 144 patients with RA-ASCVD were selected in this study, and their gender, age,RA duration, and ASCVD duration were not significantly different between different TCM syndrome types(p>0.05).The proportion of TCM syndrome from high to low was:cold and dampness syndrome(29.86%)>damp and heat syndrome(19.44%)>sputum stasis syndrome(18.06%)>Qi and Yin deficiency syndrome(14.58%)>Liver and kidney deficiency syndrome(13.89%)>rheumatism syndrome(4.17%).3.The DAS28 score,RF,ESR,CRP,immunoglobulin(IgA,IgG,IgM)were not statistically different between the types(p>0.05).4.The differences between TC,LDL,and TG levels were statistically significant(p<0.05).Among them,TC level increased most significantly in sputum(5.43±0.77mmol/L);LDL level increased most significantly in dampness(3.15±0.90mmol/L),and TG level increased most significantly in sputum(89.42mmol/L).No significant difference in HDL levels was observed(p>0.05).5.The blood uric acid(UA)level was statistically different among the types(p<0.05),with the most significant increase(331.65±112.25umol/L).No blood glucose(GLU)levels(p>0.05).6.The prevalence of dyslipidemia was statistically different among the syndrome types(p<0.05),among which the proportion of patients with dampness-heat syndrome and phlegm-stasis syndrome was significantly higher than that of qi and Yin syndrome(p=0.004,p=0.008,p<0.05).There was no statistical difference between hypertension and diabetes mellitus(p>0.05).Conclusion:1.RA-ASCVD patients have more TCM syndrome types with cold and wet obstruction syndrome.2.Age,gender,duration of RA and ASCVD are difficult to serve as reference indicators to guide the syndrome differentiation of this disease.3.TC,LDL,TG,and UA can be used as reference indicators to guide TCM syndrome differentiation in patients with RA-ASCVD,including the patients with high TC,LDL,and TG and high UA;DAS28 score,RF,ESR,CRP and immunoglobulin(IgA,IgG,IgM)are difficult to be used as reference indicators to guide the syndrome differentiation and classification of this disease.4.Dyslipidemia can be used as a reference basis to guide the TCM syndrome differentiation of RA-ASCVD patients.Patients with dampness and heat obstruction syndrome are more likely to have dyslipidemia.
