| Objective:To investigate the classification of molecular subtypes of muscle invasive bladder cancer(MIBC)and its relationship with prognosis, compare the survival rate, metastasis rate and recurrence rate of patients with different subtypes, and explore further treatment options for patients with different subtypes. Methods:From January 2015 to December 31 2022,122 patients with muscle invasive bladder cancer in the Affiliated Hospital of Qingdao University were retrospectively analyzed and followed up.According to the immunohistochemical results, these patients were divided into three types:lumen type(CK20+,GATA3+),basal type(CK14+,CK5/6+)and mixed type(CK14+,CK20+).All kinds of past medical histories of patients were collected, including whether they had diabetes, hypertension, and smoking history,Do you have a history of taking statins.Summarize and analyze the above data to compare the overall survival(OS)and progression free survival(PFS)of patients with different classifications,as well as the survival, metastasis, and recurrence rates of patients with three subtypes up to the end of follow-up.Result:Among the 122 patients, there were 26 basal type,72 luminal type, and 24 mixed type. All three subtypes were correlated with OS,PFS,and overall survival rate(P<0.05).The luminal type had the longest OS and PFS,and had the highest survival rate;Compared with the other two types, the recurrence rate of the lumen type has a statistical difference(P<0.05),and the overall recurrence rate is the lowest. There is no statistical difference in recurrence rate between the mixed type and the basal type(P>0.05);Compared with the other two types, the mixed type had a statistically significant difference in metastasis rate(P<0.05),with the highest distant metastasis rate. There was no statistically significant difference in distant metastasis rate between the lumen type and the base type(P>0.05).Conclusion:Among the three types of muscle invasive bladder cancer, the tubular bladder cancer has the best prognosis, the longest OS and PFS,the highest survival rate, and the lowest recurrence rate; The mixed type has the highest distant metastasis rate, the lowest survival rate, the shortest OS and PFS,and the worst prognosis. However, there is no statistical difference in recurrence rate between mixed type and basal type patients, and a larger sample size may be needed for further research. |
