Social Work Practice Research On Home Based Elderly Care In H Community Under The Mode Of Combination Of Medical Care And Nursing Care

With the acceleration of the aging process in society,the country is paying more and more attention to the construction and development of elderly care services.Whether it is the high-quality development of the 14 th Five Year Plan,or the strategic priority given to ensuring people’s health in the report of the 20 th National Congress and the implementation of the national strategy to actively respond to population aging,all sectors of society are called upon to pay attention to the issue of population aging and jointly promote the development of elderly care.At present,community home-based elderly care,as a type of elderly care model,has been actively advocated.However,in terms of its effectiveness,professional intervention is still needed to promote the significant development of community home-based elderly care services.Based on this,this article takes the intervention of social work in urban community home care services as the research content,attempting to explore its service mode and strategy under the model of combining medical care and elderly care.This paper takes the community home-based elderly care service carried out by J social work agency in H community of Kunming as an example,and studies the social work practice of community home-based elderly care under the mode of combination of medical care and elderly care through literature research,in-depth interviews,participatory observation and questionnaire survey.The research content includes: firstly,introducing the research background and significance of the rapid development of aging,domestic and international research on the combination of home care and medical care,and the research ideas,methods,and innovations of the paper.Secondly,the combination of medical care and elderly care at home in communities is described as the key concepts of this paper,and the Ecological systems theory and social support theory are the theoretical support of this paper.Thirdly,an overview of the institution and the H community it serves,as well as the elderly population in the H community,was introduced.It was found that there are problems with a single content of elderly care services and insufficient hardware facilities in the community.Fourthly,it introduces the practical content of H community home-based elderly care service under the mode of combination of medical care and elderly care.Fifthly,the process evaluation and result evaluation of the practice of community home-based elderly care under the mode of combination of medical care and elderly care were carried out.The evaluation found that the community home-based elderly care service team was not mature enough,and the cultural activities of "support" were better than the support platform of "medical care".Sixthly,think about and analyze the problems existing in the community home-based elderly care social work under the combination of medical care and elderly care model,including the difficulties in linking community medical care and elderly care resources,inaccurate social work services,insufficient community participation in home-based elderly care services,and many difficulties in serving the elderly.At the same time,through research papers,propose four solutions: integrating existing resources,linking external resources;Refined classification of requirements and provision of diverse services;Introduce professional talents and stimulate community motivation;Collaborate with community forces to promote smart elderly care.Through the research of this paper,the author believes that medical social work and community home-based elderly care can be integrated and developed.The social work practice of community home-based elderly care under the combination of medical care and elderly care mode has the characteristics of humanization,accommodation,integration and development,which is more conducive to the construction and development of community home-based elderly care.
