Correlative Study On Evaluation Index Of TCM Effectiveness Of Chronic Heart Failure

BackgroundChronic Heart Failure (CHF), is the terminal stage of various cardiovascular diseases. The validity of the evaluation of TCM is a core problem. The early group develops a disease-specific scale which could simultaneously reflect the field of CHF and features of TCM-The CHF-Quality of Life Scale of Integrated Chinese and Western Medicine, and it provides a practical and reliable tool for curative effect evaluation.ObjectiveStarting with the result of The Research in the Chronic Heart Failure-Quality of Life scale of Integrated Chinese and Western Medicine and Universal TCM Diagnosis and Treatment of Chronic Heart Failure Community Project, this research notice the connection among the QOL scale, TCM symptoms scoring scale and modern medicine correlation between physical and chemical index and primary outcome.Methods(1) The method of scale development. On the base of self-created scale in Universal TCM Diagnosis and Treatment of Chronic Heart Failure Community Project, scientific research projects:research of homemade symptoms score scale, on the basis of the discrete trend method, correlation coefficient analysis method, option reaction degree analysis, cronbach’s alpha coefficient method for56filtered scale items at the beginning, and to evaluate the reliability, validity and reaction degree of the official scale.(3) The method of Pearson correlation coefficient and Spearman rank correlation. The research.notices the correlation the4indicators with The CHF-Scoring scale of symptom and The CHF-Quality of Life scale, including the NT-proBNP, LVEF, cardiac function (NYHA) classification,6MWT walking distance.(4) The method of analysising the correlation between the curative effect evaluation index and cardiovascular event.The method of grouping, which means grouping the cardiovascular events (Hospitalized again, Acute Coronary Syndrome, malignant arrhythmia, death) with Independent-samples T test or wilcoxon tow-sample test and binary logistic regression analysis.Results (1)36items were screened in The CHF-Scoring scale of symptom in TCM, the response rates of which were all higher than85%, The cronbach’s alpha reliability is0.924, higher than0.8; the split-half reliability is0.897, higher than0.7. The scale and cardiac function NYHA classification spearman correlation coefficient is-0.413, P<0.01, which shows has good reliability and validity. The correlation coefficient results show the scale has good content validity. The analysis of principal component factor method proved that the scale of Construct Validity was good.(2) The results of paired-samples T test and wilocoxon paired test for LVEF, NT-proBNP,6-MWT walking distance, the CHF-QOL scale, CHF-scoring scale of symptoms, showed that before and after the other four curative effect evaluation indexes are significant difference, the LVEF results subgroup analysis, found the LVEF in patients with40%or less significant differences before and after therapy.(3) The results of analysis of correlation between the score of CHF-Scoring scale of symptom and CHF-Quality of Life scale, with LVEF, NT-proBNP,6MWT walking distance, cardiac function NYHA classification, showed that there was significant correlation between the index has significant correlation between the rest except the LVEF,6MWT walking distance and the score of CHF-Scoring scale of symptom.(4) The results of statistical analysis of the score of CHF-Scoring scale of symptom and the score of CHF-Quality of Life scale, and LVEF, NT-proBNP,6MWT walking distance, cardiac function NYHA classification and cardiovascular events for6months,Showed that all the index are associated with6months cardiovascular events, except the6MWT walking distance.Conclusions:CHF-Scoring scale of symptom in TCM has good reliability, validity and reaction degree, clinical operation is feasible and can be accepted by patients. There are significantly correlation among CHF-Scoring scale of symptom in TCM and CHF-Quality of Life scale of Integrated Chinese and Western Medicine, and modern medicine recognized LVEF, NT-proBNP,6MWT walking distance, cardiac function NYHA classification and cardiovascular events, it would be used as curative effect evaluation index for heart failure combined with traditional Chinese and western medicine.
