Vascular dementia (VD) is a generic name of chronic brain syndrome caused by damageof brain tissue which results from a series of cerebrovascular blood-supply disorder. VD,occurring on the basis of cerebrovascular disease, is an acquired durative mental impairmentdisease characterized by functional impairment of memory and cognition, or accompanied bydisorder of language, vision to space, emotion and personality. The incidence of VD is secondonly to Alzheimer's disease(AD) among the old dementia-stricken people, more and morebecoming common disease and frequently-occurring disease of influencing middle-aged andaged people's health and quality of life. And with the development of Chinese agedpopulation , It will bring heavy burden to society and family. For the treatment of VD, there isnot effective methods and medicine now. It is significant for adopting Chinese medicine totreat VD. Although there is not VD's name in traditional Chinese medicine(TCM), VD belongs to"liking forgetting disease", "foolish disease", "disordered language disease", "depressedsyndrome", "insane disease", "psychosis", "dementia" in TCM age-old record. From hislong-term clinical practice and VD's pathogenesy of modern medicine and TCM's documentsof recent years, professor Li-Shi-Mao thinks that interties of phlegm and blood stasis (IPBS)isthe basic pathogenesis of VD, that the therapeutical principle of VD should be eliminatingpathogen and that the therapeutical method should be promoting blood flow and dissipatingphlegm. In order to investigate mechanism of treating VD using promoting blood flow anddissipating phlegm(RLPBDP), we have studied the mechanism of treating VD by RuanmaiCapsule representing the method of RLPBDP. This paper is made up of three parts: The first part is document overview. Only by studying on VD's recognition of TCM andmodern medicine, can we find a way to advance and bring new ideas. The second part is about theoretical research. From discussing historical source, conceptand formative reason of phlegm and blood stasis and the relation between VD and phlegmand blood stasis , we will discuss that homologisation of phlegm and blood stasis, IPBS is thebasic pathogenesis of VD and that RLPBDP is a fundamental way to treat VD. Throughexpounding Ruanmai capsule (including its origin of prescription, indication, efficacy,regularity of compatibility) and discussion of modern pharmacology to its Chinese crudedrug,we will reveal the mechanism how RLPBDP treat VD. The third part is experiment. Through experiment to study Ruanmai capsule made byadopting modern technology, we will investigate pathogenesis of IPBS and modernpharmacological mechanism how RLPBDP treat VD. This part is divided into in vivoexperiment and cell culturing. 1. In vivo experiment Using the mouse model with acquired memory disorder resulting from hyoscine, alcoholand aluminum chloride, we had recorded the error number in 5-minute training-time andincubation that mouse first jumped down platform and the error number in 5-minutememory-time. The result indicated that Ruanmai capsule could obviously reduce the errornumber of the model mouse with acquired memory disorder resulting from hyoscine, alcoholand aluminum chloride in 5-minute training-time and the error number in 5-minutememory-time, that Ruanmai capsule could obviously lengthen incubation that the mouse firstjumped down platform(P<0.05, or P<0.01). The result shows that Ruanmai capsule hascertain preventive and therapeutic effect on VD. In many ways Ruanmai capsule can protect the rats of ischemic reperfusion. For the ratsof ischemic reperfusion, Ruanmai capsule can heighten the activity of SOD in blood and CATin pallium, lower the content of MDA in serum, thus lighten cerebral damage of oxygen freeradical (P<0.05, or P<0.01). It can also heighten NO and 6-keto-PGF1a in blood andNa+-K+-ATP in pallium and lighten the content of LA in pallium(P<0.05, or P<0.01), so it canimprove energy metabolism of brain and protect brain.
